Health Center Program - HEALTH CENTER CLUSTER - Applicant: Refuah Health Center, Inc. (RefuahHealth) is seeking support under this Service Area Competition (SAC) to continue to serve as the Health Center grantee for the proposed service area. As a recognized Level 3 Patient-Centered Medical Home, RefuahHealth serves the medically vulnerable in Rockland and Sullivan counties in New Yorkâ€™s Lower Hudson Valley. This service area spans 2,535 square miles and includes both rural and urban areas. RefuahHealth flagship location in Spring Valley, NY is approximately 35 miles north of New York City. Refuah has provided care for the areaâ€™s underserved since 1992, first receiving Section 330 (e) funding in 2002. As an FQHC, Refuah recognizes its role as the areaâ€™s primary health care provider and offers comprehensive primary care, dental, mental health, and a range of specialty services (podiatry, urology, ophthalmology, etc.). Refuahâ€™s target population includes low income residents living at or below 200% of the Federal Poverty Guidelines (FPG) and the un/underinsured. In 2022, Refuah provided care to over 78,000 low-income New Yorkers, well exceeding the patient target of 71,477. Major Health Care Needs and Barriers to Care: The target population of low resourced residents suffer from significant health care disparities, including heart disease, asthma, low birth weight, hypertension, diabetes, cancer, substance abuse, lack of adequate prenatal care, oral health, and mental health. The target population is more likely to be diag nosed with chronic conditions that necessitate routine, affordable care. Many of these disparities result from the health inequality experienced by low resourced individuals, who, due to economic and racial barriers that prevent care, are more likely to lead less healthy lives. Refuahâ€™s service area is uniquely characterized by large immigrant populations, including Hispanics and Haitians and one of the largest Hasidic Jewish communities in the US. These groups have their own health disparities and access issues including cultural differences, language barriers and low rates of health-literacy. Having served the community for over 30 years, Refuah is an expert in providing culturally and linguistically appropriate services. How Project Will Address Area Needs: In 2022, Refuah served 78,592 unduplicated patients through 273,257 clinic visits and 22,660 virtual visits. To meet area need, RefuahHealth operates three (3) permanent locations in Rockland, two (2) fixed satellite sites in rural Sullivan County, and a fleet of five (5) mobile medical vans. Over the past year the Center has opened two new sites and will open one new fixed site and one new mobile site within the coming year. Refuah also operates two additional sites in Rockland County through partnership with sub-grantee, Sun River HealthCare. Refuah employs 367.1 FTE providers and staff including, 142.55 medical staff, 38.37 FTE dental staff, 6.78 FTE mental health professionals, 6.17 FTE professional service providers (podiatry and rehabilitation), 2.49 FTE ophthalmologists/optometrists, 3.79 FTE pharmacy personnel, and 22.94 FTE enabling professionals. As a Level 3 PCMH, Refuah is committed to promoting the physical and mental well-being of area residents through integration of services, patient education and empowerment, and community outreach and involvement. SAC funding will allow Refuah to provide high-quality and culturally sensitive comprehensive healthcare services to 89,969 patients through 362,9 97 visits.
