Health Center Program - HEALTH CENTER CLUSTER - Project Title: Health Center Program Service Area Competition (HRSA-24-068) Applicant Name: Community Health Centers of Southeastern Iowa (CHC/SEIA) Website: www.chcseia.com Funds Requested: $2,844,565 SAAT Identification Number/Location: 170/West Burlington, Iowa Community Health Centers of Southeastern Iowa (CHC/SEIA) first received Health Center Program funding in 2003 to provide primary healthcare for the underserved in a rural five-county area in far southeast Iowa and west central Illinois. Over the past 20 years, we have continued to grow to meet the needs of the underserved. Today, we operate five sites in Iowa (two in West Burlington, two in Keokuk, and one in Columbus City) and one in Illinois (Hamilton). Our target population includes individuals with incomes at or below 200% FPG, the uninsured and underinsured, and those who face other barriers to care. This population experiences significant health challenges, including higher rates of diabetes, obesity, hypertension, heart disease, cancer, and behavioral health issues compared with the overall population. In 2022, we provided care for more than 23,049 patients. Twenty-five percent of patients are racial/ethnic minorities and nearly 6% are best served in a language other than English. More than 84% of patients have incomes below 200% FPG, 54% are Medicaid/CHIP recipients, and 7% are uninsured. Our long history of providing quality care for the underserved gives us a unique understanding of the needs of the target population. We provide access to comprehensive medical, dental, and behavioral health services for all ages, regardless of ability to pay. Medical care focuses on the prevention and early identification of common medical problems such as diabetes, high blood pressure, obesi ty, and heart disease. Services include annual physical exams and screenings, well child exams, immunizations, family planning, tobacco cessation, access to low-cost medications, and laboratory services. Oral health services are available for children ages six months through adulthood and include general dentistry, including examination and diagnostics, prevention education, teeth cleaning and polishing, emergency care, oral surgery, and restorative treatments. Our behavioral health providers are available on site or through telehealth to address an array of problems, including depression, mood disorders, anxiety, alcohol and drug use.? We also offer supportive services to ensure patients have full access to care, including, but not limited to, transportation, language interpretation/translation, specialist referrals and appointments, and eligibility determination and enrollment in state and federal programs, including Medicaid, CHIP, and the Health Insurance Marketplace. In 2022, our dedication to providing outstanding patient care was recognized by HRSA through Community Health Center Quality Recognition Badges for Health Center Quality Leader, Access Enhancer, Health Disparities Reducer, and Advancing HIT for Quality. Ongoing Health Center Program funding will allow us to continue providing essential primary and preventive care to the underserved in southeast Iowa and west central Illinois and to focus efforts on addressing the unique needs of our patient population. Our unduplicated patient projection for 2025 is 21,765.
