Health Center Program - Duffy Health Center, founded by volunteers and incorporated in 1997, has been a HRSA 330(h) Health Care for the Homeless program since 2002. Located in Hyannis, MA (Service Area ID: 126, Health Center Program Award No. H80CS00271), the center projects serving 3,008 unique patients in 2026. POPULATION: Duffy provides care for the most medically complex and vulnerable adults in Barnstable County, MA, which consists of 15 towns along the Cape Cod peninsula. Homelessness on Cape Cod is mainly ‘hidden,’ i.e., persons living doubled up, moving in and out of off-season rentals, or in unsafe or unstable housing. Contributing factors include a lack of affordable housing, poverty, and a seasonally influenced economy. The majority of Duffy’s patients are well below the Federal Poverty Guidelines (FPG). In 2023, slightly more than 77% of Duffy patients are under 200% of the FPG, and almost 58% of Duffy patients are under 100% of the FPG. NEEDS: In the population of people who are homeless or at risk for homelessness, there is a high rate of chronic and acute health problems, including mental health and substance use disorders. Alcohol and drug problems can be causes and consequences of homelessness; they are also co-occurring problems that complicate efforts to find stable housing. People who are homeless are at elevated risk for experiencing substance use disorders (SUDs), mental disorders, trauma, medical conditions, employment challenges, and incarceration. Duffy patients reflect this medical complexity: 43% of Duffy clients have a substance use disorder (SUD) and 21% have an opioid use disorder (OUD); 58% of clients have a mental health diagnosis; and 35% have both a mental health diagnosis and SUD. Risk factors intensified during the COVID pandemic with an increase in premature deaths, especially of young adults, from opioid use. Cape Cod is a state-designated ‘hot spot’ for overdose deaths and for hepatitis C, which is a comorbidity of SUD. SERVICES: Duffy's clients have often encountered stigma and barriers to care. Duffy staff overcome these barriers using a nonjudgmental, patient-centered approach that emphasizes trauma-informed care (TIC), which recognizes the widespread impact of trauma, avoids re-traumatization, and promotes healing and recovery. The team approach begins with outreach, moves to engagement, and through constant support makes a positive impact on patient health. Duffy Health Center’s services include primary care, psychiatry and mental health services, SUD treatment and peer recovery support, case management (includes housing support services), and spiritual care. Emerging from the pandemic, Duffy Health Center has expanded telehealth appointments, increased psychiatry and prescribing services (e.g., for office-based addiction treatment), and implemented a state-funded, evidence-based suicide prevention program. Duffy offers services in the community -- collaborating with over 20 health, housing, and social service agencies -- that includes an emergency shelter program during the winter. In August 2023, Duffy launched its REACH mobile unit, which operates a 4-day per week schedule with regular stops in five towns on Cape Cod. The REACH team offers safe, nonjudgmental outreach and harm reduction services, as well as access to treatment and recovery support, to persons who have or are at risk for SUD. In November 2023, following a three-year community wide planning initiative, Duffy introduced a state-funded medical respite program on Cape Cod that provides recuperative care services to homeless patients after discharge from inpatient settings. Duffy administers the program in collaboration with the local hospital system and community partners. In all its services and programs, Duffy Health Center improves the quality of life for acutely vulnerable populations on Cape Cod through whole person health care, case management and recovery support, and community leadership and advocacy.
