HPCHC strives to be the provider of choice for quality, accessible primary health care services in Southeast Colorado. HPCHC, located in rural Southeast Colorado, began serving patients in 1995 as a single office medical and dental facility. Throughout its history, High Plains has grown to five service sites and one administrative site, providing medical, dental, and behavioral health to residents of Prowers, Bent, Baca, and Kiowa counties. Four clinics are centralized in Lamar, Colorado, the county seat of Prowers County, and access is expanded in Holly, Colorado, also located in Prowers County. Prowers County is located in the Arkansas River Valley, offering a semi-arid climate that supports agriculture, specifically the production of corn, alfalfa, wheat, and livestock. The industries of Agriculture in Prowers County is second only to Healthcare and Social Assistance. As such, HPCHC is designated as a migrant health center (MHC), utilizing section 330g funds to ensure the availability and accessibility of required primary health care services to migratory and seasonal agricultural workers and their families in the service area. HPCHC is requesting Service Area Competition (SAC) funding as a competing continuation applicant for a proposed three-year budget period to continue providing critically needed primary care in southeast Colorado. Prowers County is designated as both a Medically Under-served Areas (MUA's) and Health Professional Shortage Area (HPSA's), with 28.8% of the population living in families with incomes at or below 150% of the federal poverty level. Prowers County holds a poverty rate of 15%, higher than the state of Colorado rate of 9%, and national rate of 10%. Twenty-four percent of the children in Prowers County are living in poverty, which is more than double the state average of 11%, and significantly higher than the national average of 16%. Health disparities in HPCHC’s service area include high mortality rates for cancer, lung disease, kidney disease, and hypertension. County residents also experience a higher incidence of chronic disease including diabetes, obesity, COPD, and high blood pressure than the state on average. In the most recent Community Health Needs Assessment completed in 2024, the most significant health needs identified in southeast Colorado are (1) Access to Behavioral Health Services, (2) Health Prevention, Education, and Management of Disease and (3) Social Determinants of Health. These priority areas coupled with the economic realities of the region’s residents make access to HPCHC’s affordable primary medical, dental, and behavioral health a regional community priority. In 2023, the health center served 7749 patients with its integrated services including primary and oral preventative care, mental health, family planning, radiology, low-cost prescriptions, community education, telehealth, women and family services, same day appointments, after hours coverage, and enabling services. HPCHC will continue offering culturally and linguistically competent primary medical, dental, and behavioral health to the residents of southeastern Colorado. The health center’s total number of unduplicated patients projected to be served each year in 2025, 2026, and 2027 is 7749.
