(NEON) is now in its fifth decade of service as a Community Health Center organization. Demonstrating a long-standing commitment to promoting health and access for communities of need, 2024 marks 57 years of service to Greater Cleveland residents. NEON’s mission is to enhance the quality of life of Northeast Ohio residents by providing comprehensive and excellent health care services in a culturally sensitive and caring environment. Over its history, NEON has built a network of (11) eleven brick and mortar community health center sites, several nursing home affiliations, medical services to a Job Corp Center, two mobile units serving the medical and dental needs of the service area respectively, access to telehealth, and other outreach programs for patients across the lifespan. This includes two School Based Health Clinics (SBHC). NEON’s health centers focus on providing access to patient-centered, holistic care that seeks to reduce health disparities throughout the service area and keep patients out of the hospitals and emergency rooms. NEON’s core service area population includes 21 defined urban communities: 19 Statistical Planning Areas within the City of Cleveland, Euclid, the entire City of East Cleveland and three census tracts within the inner ring suburbs of Maple Heights. NEON’s service area population includes a majority of African Americans (46.6%). Cleveland is the second largest city in Ohio. As of July 1, 2023, Cleveland’s estimated population is 362, 656. According to the Ohio Capital Journal, as of 2022 Cleveland remains identified as number 1 among larger U.S cities with the highest poverty rates - overtaking Detroit. Children and seniors both represent traditionally economically dependent populations. Between 2010 and 2020 the estimated number of older adults in Ohio increased from 1.5 million in 2010 to 1.9 million, while 16.4% of the population under age 65 suffers from a disability. The average income in the past 12 months is merely $26,040 – rendering the region’s poverty level at 31.2% according to the 2020 U.S. census. Economic conditions have resulted in high concentration of individuals living in poverty within the service area. Residents face a range of health disparities including cardiovascular disease, diabetes, hypertension, cancer, and poor perinatal outcomes. Service area residents and increasingly residents from the inner ring suburbs access primary care services through NEON’s network. Collectively, these neighborhoods represent areas of immense need of services and resources to improve health outcomes. The NEON service delivery model creates a Patient-Centered Medical Home (PCMH) for residents to access comprehensive services including adult medicine, pediatrics, OB/GYN, dentistry, behavioral health, optometry, podiatry, substance use disorder, enabling services, social services, nutrition services, primary health services, and preventative health services. NEON’s multi-disciplinary team of clinical providers represents internal medicine, pediatrics, and family medicine physicians, mid-level providers and dentists. In 2023, NEON served 16,785 residents and provided 46, 097 encounters. For the past 40 years NEON has continuously been accredited by the Joint Commission and is a recognized level 3 NCQA PCMH. NEON accepts Medicaid, Medicare, and commercial insurance. A sliding fee scale is available for patients who are uninsured and underinsured. NEON’s Outreach and enrollment initiatives assist patients in connecting them with coverage options made possible by the Affordable Care Act. With the continued 330 e funding, NEON will continue to build on its past successes to continue to make quality accessible care available to Greater Cleveland residents.
