Health Center Program - HEALTH CENTER PROGRAM: H80CS00174 SERVICE AREA ID NUMBER, CITY, AND STATE: ID #62; Richmond, VA PATIENTS PROJECTED TO BE SERVED IN 2026: From 01/01/2026-12/31/2026, Vernon J. Harris East End CHC, doing business as Capital Area Health Network (CAHN) projects serving 14,725 patients. DESCRIPTION OF PROJECT: Since 1998, CAHN has provided high quality, comprehensive health care services to medically underserved residents of the City of Richmond, as well as parts of the surrounding Henrico and Chesterfield counties. CAHN’s continued presence in the service area ensures that health care services, including general primary care, behavioral health, HIV care, dental care, and enabling services are available to all residents regardless of ability to pay. As a testament to the high-quality care provided, CAHN has received Patient Centered Medical Home certification. In 2023, CAHN served 15,499 total unduplicated patients, maintaining a positive three-year trend of 3.72% over 14,943 served in 2021. NEEDS TO BE ADDRESSED: Data from the service area reveal systemic, widespread health disparities and emphasize the need for comprehensive primary and preventive health services that are accessible to those with low incomes and the uninsured. Nearly a quarter (23.9% or 221,702 individuals) of residents are in the CAHN target population, or live on incomes at or below 200% of the Federal Poverty Guidelines (FPG) (ACS: 2018-2022, Census Bureau, 2023). The residents of this highly urban (95.5%) region faces unique challenges to health and well-being (Census 2020, Census Bureau, 2023). Approximately 70% of CAHN patients live in inner-city Richmond. All three service area regions are designated Health Professional Shortage Areas (HPSAs) and Medically Underserved Areas or Populations (MUA/P) (Find Shortage Areas, HRSA, 2024). The population to primary care physician ratio in the service area (2,458:1) exceeds the state (1,320:1) and national (1,310:1) averages; with even more severe shortages among dental and mental health providers, presenting a significant barrier to care particularly for low-income residents (HPSA Data, HRSA Data Explorer, 2024; County Health Rankings, RWJF, 2024). An estimated 17.7% of service area residents are enrolled in Medicaid, while 16.6% are enrolled in Medicare. In the City of Richmond, 9.1% are uninsured and more than one quarter of residents (25.6%) are enrolled in Medicaid, reflecting the pockets of poverty that are spread throughout the city (ACS: 2018-2022, Census Bureau, 2023). PROPOSED SERVICES: The proposed project will address the need for comprehensive primary health care services for the target population. The health center provides care from seven service delivery locations and one mobile unit. CAHN offers primary care, laboratory, screenings, coverage for emergencies during and after hours, voluntary family planning, immunizations, well child services, gynecological care, preventive dental, pharmacy, case management, eligibility assistance, health education, outreach, transportation, and translation directly. Additional services directly include additional dental, mental health, substance use disorder, nutrition, additional enabling/supportive, cardiology, and infectious disease. In Year 1, staff will include 6.6 FTE family physicians, 3.1 FTE pediatricians, 1.0 FTE cardiologist, 6.0 FTE nurse practitioners, 2.0 FTE physician assistants, 17.0 FTE nurses, 23.0 FTE other medical staff, 2.0 FTE laboratory staff, 2.0 FTE dentists, 1.0 FTE dental hygienists, 3.0 FTE other dental personnel, 1.0 FTE LCP, 1.0 FTE LCSW, 1.0 FTE behavioral health coordinator, 1.0 FTE SUD provider, 1.0 FTE pharmacy staff, 6.0 FTE case managers, 3.0 FTE outreach staff, 3.0 FTE transportation staff, and 1.0 FTE eligibility assistance staff. POPULATION GROUPS TO BE SERVED: CAHN prioritizes serving low-income individuals in the service area, including Medicaid recipients and the underinsured/uninsured.
