RMCL is a private, non-profit corporation with 501(c)3 status governed by a volunteer board of directors from service area towns. RMCL offers sliding fee discounts for low-income patients and care management support to help patients access additional services. RMCL has achieved NCQA recognition as a Patient-Centered Medical Home (PCMH) for both its Lubec and East Machias clinical locations. RMCL serves a rural, geographically isolated population in Washington County, Maine -- the easternmost county in the United States. Washington County has lower income and educational attainment and higher rates of those living in poverty or with a disability than other Maine counties, with much of the population at or near retirement age. Our service area includes 14 towns and unorganized territories with populations ranging from 400 to 1,600 people, for a total service area population of approximately 10,051. The driving distance from Lubec to the nearest critical access hospital is 30 miles and the closest tertiary care hospital is 115 miles away. Health care needs for the region are reflected in the following adverse health indicators (2022 Maine Shared Community Health Needs Assessment [CHNA] for Washington County and Maine): Washington County Maine Individuals living in poverty 18.9% 11.8% Median Household Income $ 41,347 $ 57,918 Unemployment Rate 6.2% 5.4% Persons with Disability 22.5% 16.0% Associate degree or Higher 31.2% 41.9% Uninsured Individuals 12.1% 7.9% Staffing for RMCL includes 5.16 FTE medical providers, 1 FTE Dentist, 1 FTE Dental Hygienist and 3 FTE Mental Health and Substance Abuse Counselors. Comprehensive medical, substance abuse/mental health counseling, and support services are offered at both Lubec and East Machias Clinics; comprehensive dental care is offered at the Lubec clinic. In FY 2023, RMCL provided 18,591 total visits, 11,421 medical visits, 2,822 dental visits and 2,859 behavioral health visits for 3,346 patients. RMCL projects to serve 3,346 unduplicated patients in Calendar Year 2026 and is requesting Competing Continuation funding in the amount of $1,568,277.
