In 1972, CHCSCT became incorporated in Gonzales County. The organization has eight locations within its five-county service area including Gonzales, Guadalupe, Caldwell, Bastrop and Victoria County. Currently, Gonzales and Guadalupe County continue to be designated as Medically Underserved Areas (MUA). In terms of the population, in 2023, 460,685 persons lived within service area (US census bureau Quick Facts 2023 estimated), as compared to 2019 when the service area population was 412,155. Presently, the distribution for the service area is Caldwell County 11%, Gonzales County 4%, Guadalupe County 41%, Bastrop County 24% and Victoria County 20%. The population distribution indicates an increase of 12% in the areas served. For the combined service area, 73% of the population served is at or below 200% of the federal poverty level (UDS Data 2023) and 20% of the total population was reported as uninsured (US Census Bureau 2023). Across the five-county service area, there is a higher prevalence in health risk factors including adult smoking, obesity, physical inactivity and teen births compared to the state level. On average, there is also a lower life expectancy within the service area compared to the State. (County Health Rankings 2023). In addition, there are prominently higher rates for diabetes and hypertension compared to both the State and national level (UDS Data 2023). Given the evident health disparities across the service area it is essential for the population to continue to have access to high quality and affordable care through a Federally Qualified Health Center. Through this project, CHCSCT is proposing to continue supporting the need in the communities served through providing primary medical, dental and behavioral health, gynecological and obstetrical care, substance abuse, inpatient care, diagnostic lab, specialty care referrals, health education, virtual care, WIC, eligibility assistance, outreach and case management. To ensure support for the scope of work, CHCSCT currently supports 196 full time equivalents (FTE) which consist of 57 medical (20 Providers and 37 clinical support staff including nurses, laboratory and other medical personnel) Dental staff includes 19 FTE’s (6 dentists, 3 dental hygienist and 10 dental assistants). In addition, there are 5 mental health staff, 4 enabling services, 12 other programs/services, 54 administrative and 46 patient support staff FTE’s (UDS Data 2023). During 2023, CHCSCT served 19,938 unduplicated patients. CHCSCT reported 43,637 medical, 9,678 dental, 3,769 mental health, and 126 enabling encounters for a total of 57,210 encounters (UDS Data 2023). CHCSCT projects to serve 19,938 unduplicated patients. The goal for CHCSCT according to the Board approved strategic plan is to expand the organization’s role in the region as the primary provider of health care services for all members of the communities served.
