Health Center Program - HEALTH CENTER PROGRAM: H80CS00096 SERVICE AREA ID NUMBER, CITY, AND STATE: ID # 277; Mantachie, MS PATIENTS PROJECTED TO BE SERVED IN 2026: From 01/01/2026-12/31/2026, Mantachie Rural Health Care Inc. (MRHC) projects serving 4,975 total unduplicated patients. DESCRIPTION OF PROJECT: Mantachie Rural Health Care, Inc. (MRHC) is a Federally Qualified Health Center (FQHC) serving Itawamba, Lee, Monroe, and Prentiss counties in northeastern Mississippi, near the Alabama-Mississippi state line. MRHC has provided services to this rural region of the state for more than 60 years. The health center operates six fixed locations and a mobile unit to ensure access by local residents. In 2023, MRHC served 4,975 patients with 10,101 medical, 3,804 dental, and 3,557 mental health encounters (UDS, 2023). MRHC has experienced a positive growth trend, with an increase of 26.2% in the number of patients between 2021 and 2023. MRHC has received recognition from the National Committee on Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH) and is eligible for priority points as part of this SAC application. As an existing FQHC with an established presence in northeastern MS, MRHC is well-positioned to continue providing access to comprehensive primary and preventive health services for the low-income residents of Itawamba, Lee, Monroe, and Prentiss counties. NEEDS TO BE ADDRESSED: Data from the service area reveal systemic, widespread health disparities and emphasize the need for comprehensive primary and preventive health services that are accessible to those with low incomes and the uninsured. Within the service area, 35% of individuals live with incomes at or below 200% of the Federal Poverty Guidelines. The population to 1.0 FTE provider ratios in the service area demonstrates a severe shortage of primary care (8,347:1), dental (8,483:1), and mental health (27,782:1) providers, presenting a significant barrier to care, particularly for low-income residents (County Health Rankings, Robert Wood Johnson Foundation [RWJF], 2024; HPSA Data, HRSA, 2024). The UNS for the service area is 66.7, indicating a high level of unmet need (Unmet Need Score Map Tool, HRSA, 2024). The mortality rate for all causes is 1,019.2 in the service area; this rate exceeds the state average of 948.6 and the national average of 747.6 (WONDER, CDC, 2016-2020). PROPOSED SERVICES: MRHC operates a medical clinic, dental practice, a behavioral health clinic, an administrative site, two school-based clinics, and a mobile unit. MRHC provides primary medical, behavioral, and dental health care, along with enabling services, to meet the needs of the service area’s identified lack of access. MRHC’s service delivery model includes direct provision of general primary medical care, diagnostic lab and x-ray, screenings, coverage for emergencies during and after hours, voluntary family planning, immunizations, well child services, gynecological care, preventive and additional dental care, pharmaceutical services, mental health, substance use disorder services, and nutrition services. Directly provided enabling services include case management, eligibility assistance, health education, outreach, and transportation. Collaborative relationships provide referral access to other services such as prenatal, intrapartum and postpartum care, optometry, and translation services to which patients would otherwise have limited access. Proposed staffing to support project implementation includes 0.7 FTE physician (IM), 6.0 FTE nurse practitioners, 3.8 FTE nurses, 3.5 FTE medical assistants, 1.3 FTE dentists, 1.0 FTE dental hygienist, 3.1 FTE dental assistants, 1.8 FTE LCSWs, 1.0 FTE psychiatric nurse practitioner, 1.0 FTE Nutritionist, 0.25 FTE Outreach, 0.5 FTE transportation staff, and 1.0 FTE Quality Improvement staff. POPULATION GROUPS TO BE SERVED: MRHC prioritizes serving low-income individuals in the service area, including Medicaid recipients and the underinsured/uninsured.
