Health Center Program - With this competing continuation proposal, Delta Health Center (DHC)—the recipient of Health Center Program Award H80CS00085 since 1992—seeks to serve 100% of 14,327 target patients listed in the HRSA Service Area Announcement Table (SAAT) for Service Area Competition ID 093. This is the actual number of unduplicated patients served by DHC in CY2023. DHC is the only FQHC with at least one access point in each of the six sparsely populated, rural counties comprising the service area, which lies in the heart of the Mississippi Delta. Most area residents are Black (71.3%), and many (30.2%) live in poverty. DHC serves some of the poorest and most underserved of these residents: 94.1% of our patients are Black and 81.0% live in poverty. Founded in 1965, DHC was the first rural health center in the US. The Delta at that time was plagued by abysmal poverty, wide unemployment, crumbling shacks, outright malnutrition, and contaminated water, causing many preventable deaths and much suffering. The infant mortality rate, at 60 deaths per 1,000 live births, approached that of third world nations. From its inception, DHC rapidly emerged as an icon of compassion and healing, gaining the trust of its patients, who had long been neglected and desperately needed medical care. Over the last six decades, DHC has grown from its original site in Mound Bayou to 11 strategically located primary care access points, two dental clinics, and five school-based health centers. DHC has also made great strides in improving the health of Delta residents; yet much remains to be done. Compared to Mississippi and the US, the DHC service area exhibits higher rates of teen birth, low birthweight, obesity, sexually transmitted infections, and chronic diseases including diabetes, heart disease, and asthma. Less than half of service area residents have had dental care in the past year, and 18% report that their mental health was not good for more than two weeks during the past month. The mission of DHC is to provide comprehensive, high quality, primary healthcare in a respectful, culturally sensitive manner, regardless of ability to pay. To achieve this mission, DHC provides all of the required medical services on Form 5A at each primary care access point. Additional dental services are available at select locations, as well as behavioral health services, nutrition, and physical therapy. Locally, the health center coordinates services with local hospitals, county health departments, specialty care providers, and public and private providers of behavioral health services. Nationally, DHC collaborates with the Tufts University Food is Medicine program and the Partnership for a Healthier America to advance food equity and reduce obesity. To further enhance services availability and accessibility, DHC is equipping a mobile health unit, funded by The W. K. Kellogg Foundation, to provide both primary care and oral health care in Sunflower County. DHC routinely assesses patients’ social determinants of health including, but not limited to, housing, food, employment, education, security, transportation, childcare, and social support. The health center has developed extensive linkages with local governmental and nonprofit organizations to help patients meet their needs and to improve public health. In addition, DHC is now implementing CareMessage, a platform for customized text messaging used to improve patient communications. To optimize our response to the local needs, the demographic composition of DHC’s governing board reflects the community, and the health center is assembling a Community Advisory Group for each primary care clinic. DHC employs the NextGen electronic health record and practice management system, which supports clinical care coordination, practice management, and data analytics/reporting, including continuous quality improvement. The health center’s two most recent Operational Site Visits have yielded no adverse findings.
