Health Center Program - HEALTH CENTER CLUSTER - Website: www.familyhealthnetwork.org Grant Program Funds Requested: Section 330 (e) - CHC $2,300,207 Proposed Service Area ID Number, City & State: 322, Homer, NY Unduplicated Patients projected to be served in CY 2025: 15,606 Established in 1972, Family Health Network of Central New York, Inc. (FHN) is a licensed Article 28 Diagnostic and Treatment Center, consisting of 5 Health Centers, 1 that includes dental services, plus an additional 4 school-based health centers (SBHC). FHN is the only source of healthcare in 4 of the communities served, and the only source of primary care on a sliding fee scale for qualifying individuals and families in Cortland County and contiguous regions. FHN operates a Mobile Medical Unit that delivers essential health care to a homeless population and to areas where there is limited access to primary care. All Health Centers are located in Health Professional Shortage Areas (HPSAâ€™s) and serve a rural, impoverished population, with a significant percentage of individuals living below 200% of the federal poverty level (FPL). Of the known patient demographics, 68% of FHNâ€™s patients fall below 200% of the FPL. FHNâ€™s HPSA designation for medical, dental, and mental health are 19, 24, and 22, respectively. FHN is the only organization in Cortland County with a mission to provide quality primary care with a focus on the medically underserved. FHN accepts all patients, regardless of payer source or ability to pay. Regional health assessments demonstrate significant health issues that impact our target population, many of them preventable or treatable with early access to primary care. In the FHN Central NY region, the incidence of lung and bronchus cancer is 83.3/100,000 compared to 55.4/100,000 for New York State (NYS); with a mortality rate of 47.2/100,000 compared to 29.5/100,000 for NYS. This statistic is attributable to the fact that 18% of adults are smokers, compared to 12% for NYS. Diabetes mortality rates are 28.1/100,000 vs. the NYS average of 21.6/100,000. Cortland County experiences a high prevalence of drug and opioid abuse. The rate of emergency department visits that involve drug overdose is 270.8/100,000 compared to the NYS rate of 174.9/100,000. In Cortland, the rate per 100,000 of patients who received at least one buprenorphine prescription for opioid use disorder is 853.5 compared to the NYS rate of 427.0. Similarly, CNY is experiencing high rates of patients in need of mental health services. Cortland County has a lower rate per 100,000 population of mental health providers than NYS at 147.4 compared to 187.2. Additionally, this service area and population have significant social care needs that include: transportation, child care, healthy food options, etc. All FHN locations are in medically underserved areas and the funds received from this award help ensure the ability to sustain operations and provide the preventative & primary care to the communities and populations served. FHN employs 5 physicians (1 of only 2 OB/GYNs in the county), 1 dentist/2 dental hygienists; 11 APPs; and 3 LCSWs. As a Silver Badge HRSA Quality Leader, FHN provides high quality, comprehensive primary care and dental services directly, and by referral. Services offered by FHN include family and general practice, pediatrics, OB/GYN, behavioral/mental health, occupational health, family dentistry, medical and dental school health, substance use disorder, care coordination, 340B pharmacy, and free insurance enrollment by our Certified Applications Counselor. As a result of this funding, FHN is able to provide care regardless of ability to pay, including mission-driven service lines such as dental, school-based care, and Mobile Medical Unit opportunities, etc. that meet the needs of the underserved in our community.
