Health Center Program - HEALTH CENTER CLUSTER - Applicant Name: City of New Orleans Health Department Type(s) of Section 330 Funding Requested: HCH Amount Requested: $2,660,267.00 The New Orleans Health Departmentâ€™s Health Care for the Homeless (HCH) was established as a Federally Qualified Health Center (FQHC) in 1988 to provide accessible and comprehensive care in the Greater New Orleans (GNO) area, which includes Orleans, Jefferson, St. Bernard, Plaquemines, and St. Tammany parishes. HCHâ€™s mission is to improve the overall health and well-being of our cityâ€™s homeless individuals, regardless of their ability to pay for services. HCH emphasizes a multi-disciplinary approach to servicing the homeless, uninsured, and underinsured persons, by combining proactive street outreach with integrated systems of primary care, dental care, mental health, case management, client advocacy, and other enabling services. HCH is committed to continuing services as one of the few health centers in New Orleans providing primary care solely for persons experiencing homelessness. Chronic homelessness is a major public health issue that has serious ramifications on overall health status and life expectancy. Accessing primary care remains one of the top issues faced daily by adults and families experiencing homelessness, and they often use emergency departments as their most accessible source of care. New Orleans is one of the nationâ€™s leading homeless cities, with a total of 1,214 people experiencing homelessness, including 364 living on the street in 2022. Compared to the public, people experiencing homelessness have higher rates of early mortality and increased morbidity. People experiencing homelessness are particularly in need of access to quality health care and require effective linkage to care. HCH seeks to mitigate t he effects that the social determinants of health can have on homeless personsâ€™ health and link them to care. Through the Service Area Competition (SAC) funding opportunity, HCH will increase healthcare access points by expanding medical, dental, and behavioral health services in facilities frequented by people and families experiencing homelessness. SAC funds will also support quality improvement activities, enhancements to surveillance data and monitoring, and Electronic Health Record (EHR) optimization to help highlight disparities in populations at increased risk. Ultimately, HCH seeks to empower people experiencing homelessness and those at risk of homelessness to access comprehensive healthcare, including chronic disease management, behavioral and mental health services, social services, counseling and medication management, health education and literacy, patient peer mentoring, as well as assistance with transitional housing, rapid rehousing, and rapid resolution. HCH clinic sites are strategically placed in areas that are densely populated with people and families experiencing homelessness and in need of preventative care. For 35 years, HCH has been providing comprehensive care to the homeless in areas identified as high risk for chronic conditions as well as co-infections and co-morbidities. In 2022, HCH served 2,205 patients with over 6,700 visits. Due to the distinct need in New Orleans, Louisiana, the health center will serve the required projected 4,575 unduplicated patients in the proposed service area ID #105 by December 31, 2025. To ensure the cityâ€™s unhoused population improves their health risk factors, HCH is seeking SAC funds to scale up its service provision while minimizing barriers to available services. HCHâ€™s interdisciplinary team will work diligently with each patient to provide individualized care plans and link patients with specialists and other needed services.
