Extended Reach MOUD (Medications for Opioid Use Disorder) - My Health My Resources (MHMR) of Tarrant County will implement Extended Reach MOUD, integrating Medications for Opioid Use Disorder (MOUD), behavioral health treatment, and recovery support for adults with Opioid Use Disorder (OUD) across Tarrant County, Texas, home to approximately 2.25 million residents. Leveraging its Certified Community Behavioral Health Clinic infrastructure and a Medication First approach, the five-year program will reach approximately 800 unduplicated individuals through outreach and screening, identify 600 as needing MOUD services, and enroll 270 in grant-funded treatment, scaling from 125 reached in Year 1 to 200 by Year 4. The population includes working-age adults 18 to 64, veterans, and individuals facing polysubstance use, chronic health conditions, trauma histories, poverty, unemployment, unstable housing, and justice-system involvement that elevate overdose risk. Core strategies include peer-led outreach, rapid screening, DSM-5-TR diagnosis, FDA-approved MOUD including buprenorphine and long-acting injectables, referral to opioid treatment programs for methadone, Motivational Interviewing, Seeking Safety, peer recovery support, and naloxone distribution. Objectives include 85% completing screening within 72 hours, 90% receiving treatment plans within seven days, and 60% remaining in care for six months.
