Strengthening Vermont’s Statewide Recovery Support Network - Recovery Partners of Vermont will implement Strengthening Vermont’s Statewide Recovery Support Network, a three-year initiative to strengthen 12 peer-led recovery organizations and connect peer recovery support with healthcare, treatment, outreach, justice, and recovery-housing systems. The project will build durable statewide organizational, workforce, data, financing, and referral infrastructure while preserving local governance and responsiveness. Project name and geographic area: Strengthening Vermont’s Statewide Recovery Support Network will operate across Vermont’s 14 counties from September 30, 2026, through September 29, 2029. Population of focus and projected reach: The ultimate population is Vermont adolescents, transition-age youth, adults, and older adults of all genders who have substance use disorders, seek recovery support, or are affected by another person’s substance use. Direct capacity-building participants will include recovery-organization leaders, board members, peer staff, recovery-residence staff, people with lived recovery experience, and healthcare, treatment, outreach, justice, housing, state, and community partners. The project will engage all 12 recovery organizations annually, at least 60 individual participants annually, and at least 120 unduplicated participants across three years. Strategies and interventions: RPV will establish a statewide steering group and three workgroups; complete organizational-capacity assessments and improvement plans; strengthen workforce supervision, retention, wellness, and leadership; develop shared recovery-housing referral and continuity resources; implement common data definitions, referral measures, dashboards, and Plan-Do-Check-Act improvement; and advance Medicaid readiness and diversified sustainability. PRCAC will lead a rural Healthcare-to-Recovery Pathway that includes older Vermonters and connects healthcare and community-care settings with peer recovery support. Springfield will lead a Justice-to-Recovery Pathway connecting corrections, probation and parole, courts, outreach, treatment, healthcare, community recovery, and recovery-housing partners. Goals and measurable objectives: Within 90 days, RPV will establish governance and workgroups. By Month 9, all 12 organizations will complete capacity assessments and improvement plans. RPV will provide at least four statewide learning activities annually, and by Year 3 at least 80 percent of organizations will document progress on 75 percent of priority actions and improve in two capacity domains. By Month 9, RPV will establish a minimum dataset and shared referral measures; pilots will test tools by the end of Year 1; quarterly dashboards will begin in Year 2; and two value briefs will be published. By the end of Year 1, RPV will complete a statewide sustainability and Medicaid-readiness action plan; by Year 3, each organization will have a sustainability plan and at least 75 percent will implement a strengthened financing, contracting, partnership, or revenue practice. Both pilots will launch by Month 6; by Year 3, each will establish at least two strengthened pathways and increase documented referrals, warm handoffs, or successful connections by 25 percent over its Year 1 baseline. A statewide replication toolkit will be disseminated by Month 33.
