Rooted in Recovery - Rooted in Recovery, led by The Recovery Den, Inc., will expand free, peer-led recovery support for adults ages 18 and older with substance use disorders and co-occurring substance use and mental disorders across Walker, Fayette, Marion, Pickens, Lamar, and Winston Counties, Alabama, serving 6,750 unduplicated adults over five years from a hub in Walker County, identified as having the nation's fifth-highest per-capita overdose rate. Participants often face unreliable transportation, unstable housing, felony records, employment gaps, and difficulty navigating treatment systems. The Recovery Den, Walker County's first and only peer-run Recovery Community Organization, served 921 unduplicated people through 2,498 visits in 2025 and will expand peer mentoring, individualized recovery planning, transportation coordination, jail-to-community assistance, family support, overdose-prevention education, naloxone access, and a workforce pathway serving 30 to 75 participants annually using evidence-based and evidence-informed practices including Peer Recovery Support Services, Motivational Interviewing, and recovery capital-building approaches. Annual service targets grow from 1,100 unduplicated adults in Year 1 to 1,500 in Years 4 and 5, with at least half of workforce-pathway participants completing a selected milestone within six months and outcomes tracked through Substance Abuse and Mental Health Services Administration (SAMHSA) Performance Accountability and Reporting System (SPARS) reporting to support continuity during treatment, reentry, and long-term recovery.
