LifeLong proposes enhanced services for 150 pregnant/postpartum women receiving residential treatment. In addition to SUD/COD treatment, recovery support, case management, and harm reduction services provided to the women, the program will include access to supplemental services including SUD/COD for the minor children and adult family members (fathers of their children, caregivers, and partners). Services will be provided in Alameda County, CA.
LifeLong will hire an Outreach Specialist who will conduct outreach at homeless encampments, shelters, hospitals, LifeLong and other community medical clinics, etc. with the goal of engaging the women in SUD treatment. LifeLong will use the Evidence-Based Practices (EBPs) of Motivational Interviewing, Seeking Safety, Celebrating Families!, and Nurturing Parenting Programs in its service delivery.
Goals include:
Conducting outreach activities, providing SUD/COD services including assessment, individual and group counseling, case management, and recovery supports to pregnant and postpartum women; provide supplemental services for minor children and family members (fathers of their children, partners, caregiver) of the pregnant and postpartum women; conducting local performance assessments and documentation according to grant requirements and guidelines. Planning and program activities will take into account a culturally responsible, gender responsive, recovery/harm reduction oriented, trauma-informed, equity-based approach.
Objectives include:
• Providing outreach services in the community, identifying women who have an SUD, and linking them to treatment based on their interest and eligibility.
• Providing evidence-based SUD/COD assessment and treatment services, harm reduction, recovery support services, and case management services to a minimum of 150 pregnant and postpartum women.
• Providing enhanced supplemental services for minor children and family network members (fathers of their children, partners, caregiver, etc.) of the pregnant and postpartum women, including SUD/COD counseling and brief interventions, family therapy, therapeutic childcare, and developmental assessments.
• Completing health disparities statement, uploading GPRA data, participating in evaluation activities, and compiling data for semi-annual reports
• Monitor race, ethnicity, sexual orientation, and gender identity of enrolled clients and present enrollment trends through monthly reports, allowing the team to ensure equity among the client population in real time, by the Research and Evaluation team.
