Project Name: Rural Community Paramedicine, EMS Training
Project Summary: Through SAHMSA funding, the Citizens Memorial Hospital EMS Department will be able to offer didactic training to 66 personnel members in the fields of substance use disorder, co-occurring mental health disorders, and coordinating care for rural populations; Funds will provide advanced training to 12 EMS personnel, making them eligible to receive a Community Paramedicine endorsement to their certification. Classroom and hands-on training will effectively equip EMS providers to treat patients within a rural 5-county district and integrate coordinated care services through in-home treatment, telehealth, and warm referrals.
Population to be Served: Citizens Memorial Hospital has 13 Rural Health Clinics and 7 long-term care facilities throughout 5 counties that are designated as rural or partially rural and have served this community for 40 years. The CMH EMS Department is comprised of 66 paramedics and EMTs, serving the rural counties surrounding Polk County where the hospital is located. They transport an average of 7,494 individuals each year.The target population group to be served includes individuals residing within the 5 rural county service area that utilize pre-hospital ambulatory services as their primary care resource. This population group has a need for integrative care coordination and a continuum of care strategy, especially for substance use disorder treatment resources. The underserved individuals within this region have historically suffered from poorer health outcomes and health disparities due to poverty, low socioeconomic status, uninsured/underinsured, under-educated, transportation barriers, and lack of available healthcare resources to meet their needs. Individuals often utilize Emergency Medical Services for non-emergency situations, but their health conditions still warrant medical intervention, treatment, and integrative follow-up care.
Proposed Service Region: 5 designated HRSA Eligible Rural Counties in southwest Missouri
Polk (Partially Rural, Census Tract 960200), Hickory (Rural), St. Clair (Rural), Cedar (Rural), and Dallas (Partially Rural, Census Tract 480200) Counties.
Strategies/Interventions:
• Recruit and Train EMS personnel to serve in the 5-county rural area
• Provide advanced training opportunities in Community Paramedicine through leveraging community partnerships and online resources
• Acquire training supplies, equipment, and two vehicles to be utilized in a Community Paramedicine capacity
• Provide training in substance use disorders, mental health, and care coordination to optimize medication assisted treatment resources and referrals to services
Goals and Objectives:
Goal: Expand training of EMS personnel to offer in-home primary care treatment and integrate coordinated care resources to improve the quality of health care services for rural residents.
Objective 1: Recruit and train 2 EMTs; Train 4 Paramedics; and provide advanced training through the MIH Network in Community Paramedicine for 6 Paramedics and initiate a CP program in the service area within the first 6 months of funding.
Objective 2: Provide 4 hours of training to all (project 70 individuals) EMTs and Paramedics through online and in-person resources to expand knowledge of substance use disorder and mental health disorders within first 6 months of funding.
Objective 3: Within the first 6 months of funding, train all 70 EMS personnel to provide coordinated care services and warm referrals to other hospital stakeholders and specialties for historically underserved individuals (i.e., home health, social work, SUD, mental/behavioral health, OBGYN, etc)
