LifeLong Medical Care (LifeLong) presents this proposal for the Enhanced MAT Program. Alameda County is experiencing an opioid crisis that is rapidly worsening despite decreasing opiate prescriptions and increasing access to Opioid Use Disorder (OUD) treatment. To address this crisis, LifeLong is proposing to expand and enhance its MAT service delivery in and surrounding downtown Oakland and West Oakland at four (4) of its programs. This proposal's population of focus are men and women experiencing homelessness with opiate use disorders and co-occurring serious mental illness and other substance use disorders. Oakland is home to a large unsheltered and vulnerable homeless population. LifeLong will provide access to MAT and enhanced supportive services to a minimum of 50 (prorated to 33 in year 1) individuals experiencing homelessness with OUD, and 233 in total. LifeLong will hire two licensed mental health clinicians (one of whom is the Project Director) and two Recovery Support Specialist (RSS) staff to reach the goals of increased outreach, engagement and retention, and a reduction in opiate use among MAT patients. Services provided by the Enhanced MAT Program will include individual and group counseling with a focus on co-occurring substance use and mental health disorders, recovery groups, access to tobacco cessation services, and case management that includes assisting patients with arranging and attending appointments related to benefits, housing assistance, employment, medical care, etc. In addition, program participants will receive coaching, emotional support, and access to the recovery community, e.g. self-help groups. Many of these services will occur at homeless encampments and other locations in the field. The Enhanced MAT Program will use the Evidence-Based Practices (EBP) of Motivational Interviewing, Seeking Safety and Relapse Prevention Therapy, all of which are normed for the population of focus and have been successfully integrated in programs at LifeLong.
