The Ho-Chunk Nation Behavioral Health Division will increase identification and MAT treatment of individuals with opioid use disorders (OUD) and referrals to appropriate treatment utilizing the SAMHSA Screening, Brief Intervention, and Referral to Treatment (SBIRT) model. Behavioral Health Shared Care Managers will be embedded in the medical clinic nurses stations that will conduct PHQ-9, GAD-7, and DAST-10 screenings, perform brief interventions, and refer patients to Behavioral Health Services in order to increase referrals to Behavioral Health by 50%, with a 25% increase each subsequent year. We will increase residential treatment opportunities for individuals with opioid use disorder by meeting ASAM criteria. We have identified a goal of assisting 30 unduplicated individuals yearly to engage in the Medically Assisted Vivitrol Program, with an overall goal of 150 for the entire project. By enrolling in the Vivitrol Program we anticipate a decrease in illicit opioid drug use and prescription opioid misuse at a six month follow-up, further decreasing overdose deaths. Through consistent use of the Vivitrol Program and Behavioral Health services, within two years we will achieve 80% opioid dependence abstinence, evidenced by negative urine drug screen results for opioids, buprenorphine, and methadone at the six month follow-up. We will measure the decreased opioid overdose deaths by data collections from HCN Police reports, local emergency room reports, pharmacy Naloxone reports, and community reports.
