Flushing Hospital Medical Center (FHMC), a 299-bed not-for-profit teaching hospital, has served as the principal provider of high-quality primary and mental health services to its community for nearly 140 years). FHMC will lead an initiative to expand access to MAT networkwide for patients with OUD. The program will recruit and serve adult clients with comorbid chronic disease, COD, SUD, and OUD, with a focus on women of childbearing age. This initiative will have a large footprint within the expansive integrated health system encompassing the: 1) FHMC Department of Internal Medicine; the FHMC Emergency Department (ED); all FHMC mental health clinic sites; and Ambulatory Care clinics; and the 2) JHMC Departments of Family Medicine and Internal Medicine, JHMC ED, Ambulatory Care clinics, and the 25 TJH Medical Services, PC clinic sites. Under the leadership of Principal Investigator, Daniel Chen, MD, Chairman, Department of Psychiatry and Addiction Services, FHMC proposes the following goals and objectives to serve the target population. • Goal 1. Increase the number of adults with OUD in the service area receiving MAT. Objective 1.1. By month 4 (1/30/2022) implement program participant outreach and engagement strategies to increase participation in and access to MAT. Objective 1.2. By month 4 (1/30/2022) implement a plan to routinely check the NYS “I-STOP” (PMP) and NYS “Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES)” at participant intake. Objective 1.3. Annually assess and screen a minimum of 500 participants to determine risk for OUD, SUD, COD, and mental disorders for the delivery of needed services to support recovery. Objective 1.4. Annually assess and screen 50 participants (year 1) and 75 annually (years 2-5) to determine persons meeting the diagnostic criteria for OUD relative to MAT. Objective 1.5. By the end of year 5 (9/29/2026) provide MAT using buprenorphine/naloxone in combination with comprehensive OUD psychosocial services for a total of 350 participants. Objective 1.6. By the end of year 5 (9/29/2026) increase the number of DATA waivered providers prescribing MAT for participants with OUD. • Goal 2. Prevent diversion for participants with OUD receiving MAT. Objective 2.1. By month 4 (1/30/2022) expand current MAT diversion mitigation protocols. • Goal 3. Decrease illicit opioid drug use and prescription opioid misuse with an expected 80% six-month participant follow-up rate. Objective 3.1. By month 4 (1/30/2022) implement telepsychiatry consultations and assessments for program participants. Objective 3.2. By month 4 (3/31/2022) provide peer recovery support services to improve access to and retention in MAT and long-term recovery of participants. Objective 3.3. By month 6 (3/31/2022) begin integrated service delivery with expansion to JHMC to identify, engage, and retain participants in treatment and long-term recovery. The proposed program will serve 50 participants in year 1, 75 annually in years 2-5, with a total of 350 participants over the five-year grant period.
