Sokaogon Chippewa Community Health Clinic (SCHC hereafter) will expand MAT services to include Suboxone. Patients who meet DSMV and ASAM criteria for this program will be prescribed and determined by SCHC certified medical staff. Patients will be monitored by SCHC staff through counseling, prescribing physician appointments, random drug screening, and proper medical lab testing. SCHC staff involved with this program shall have the opportunity for training, conferences, licensing, certifications, and continuing education as it pertains to the upkeep of this program.
SCHC will provide community education and outreach through hosting an event, during current annual community events, and incorporating in with other programs to include: The Cultural Committee, Mole Lake Coalition, Community Coalition of Forest County. SCHC staff will provide education materials, as well as specific materials in the areas of elders, children/teens, and pregnant woman.
These monies will assist individuals who have no insurance, or an insurance plan that will not cover Substance Abuse related treatment, to include, but not limited to: Any medical cost, including medication in regards to individual being able to receive appropriate level of SUD treatment, Residential Treatment, Day Treatment, Intensive Outpatient Treatment, Outpatient Treatment, Sober Living, and Drug Screening.
