Project Name: MMHC Tribal Opioid Response Project 2 Overarching Goal: MMHC will enhance and expand its existing Medication Assistance Treatment (MAT) program adding substance abuse treatment services. Delivery of MAT Services to 120 additional patients over the 2020 TOR grant project two-year period and provide substance abuse counseling to 40 new patients in year one of the 2020 TOR funding and 60-70 additional new patients in year two. MMHC is committed to delivering best practices, evidence based and trauma-informed care for our clients diagnosed with opioid use disorders. All four of our primary care providers and our psychiatric nurse practitioner have received their Suboxone (Buprenorphine) Data Waiver DEA licenses. We began seeing patients six months ago for opioid use disorders and currently have a handful of patients receiving care. Behavioral health is an equal component of care provided to patients who opt to receive substance abuse and/or mental health counseling services. MMHC would like to further develop and expand our medication-assisted treatment (MAT) program, as there is certainly a need in the tribal and general population that we serve. We would also like to grow our capacity to provide substance abuse counseling which includes other illicit drugs such as methamphetamine and cocaine. Service Area: MMHC’s service area is the city of Jamestown, a national designated historic town located in western Tuolumne County, California and the surrounding area. Jamestown is a rural and somewhat remote area with minimal services located in the Sierra Nevada mountain foothills. Target Population: The target population is the Chicken Ranch Rancheria tribal membership of 32 members, 840 unaffiliated Native American Indians who reside in Tuolumne County, and the low-income population that reside in and around Jamestown. Expected Outcomes: MMHC expect outcomes are: 1) conducted a second strategic planning session to plan substance abuse treatment program integration with our MAT program, particularly for methamphetamine and cocaine; 2) Hire a substance abuse counselor; 3) Develop and circulate program brochure and patient education material; 4) Expect to provide MAT services to additional 120 unduplicated MAT services to 60 unduplicated patients; 4) Expect to provide substance abuse treatment to 100 patients; and, 5) Expect that 80% of patients are retained in the program after a six month period of beginning the program.
