Services for the Underserved’s (S:US) project, La Confianza, is a person-centered and integrated approach to addressing substance use disorder and homelessness services for the Latinx population. La Confianza will be located at 516 West 181st Street New York, NY. The people we will serve at La Confianza come from neighborhoods that have some of the highest rates in NYC for substance use disorder and risk of homelessness: East Harlem (zip codes: 10029, 10035) and Washington Heights (10032, 10033, 10034, 10040). Both of these neighborhoods have the largest concentration of Latinx populations in Manhattan. S:US has long served individuals from these communties and will target them specifically for this project. La Confianza will enhance our Homebase program that provides homelessness services and will use EBPs such as Motivational Interviewing, Intentional Peer Support, and Trauma-Informed Care, and Whole Health Action Management. The La Confianza team will provide individualized treatment and recovery support, permanent housing, peer support and connection to benefits and entitlements. We will provide these services to 125 individuals per year (625 over the five-year program). La Confianza will be operational within four months of award. Program goals include: 1) Improve housing stability and decrease homelessness, 2) Increase engagement in integrated behavioral health services to address substance use, and 3) Increase sustainable treatment services for homeless individuals across Homebase services.
