Abstract
The Elmhurst Home, Inc. (EHI) SOAR - Building Communities of Recovery (BCOR) Project serves adult men and women at varying points of substance abuse (SA) recovery in Detroit, Michigan. The varying points of recovery include pre-treatment, actively engaged in treatment, and post treatment engagement. The project also works with family members to include children whose parents struggle with SA. The demographic characteristics include African American, Caucasian, Hispanic, Arab American, and Native American populations ranging in age 18-67 years old to include individuals who identify as lesbian, gay, bisexual, transgender, and questioning. The project seeks to expand access to peer recovery support services (PRSS), namely recovery coaches for 300 individuals, beginning Year 1 (Y1): 50, Y2: 125, and Y3: 125. To achieve this target, the project will assess 1700 individuals for eligibility to receive PRSS over the life of the grant i.e. Y1: 500, Y2: 600, and Y3: 600. The goals (G) are to 1) increase capacity to support individuals with SA and /or their families 2) expand PRSS to ensure access to recovery coaches and 3) enhance access to services through linkage to formal and informal community resources. The objectives (O) to achieve these goals are provided.
G1O1: By 12/30/2019, implement PRSS through collaboration with faith based entities in geographic areas with high AOD and low SES as evidenced by MOU with churches and # of services provided. Establish MOU with churches and deliver onsite.
G1O2: By 7/30/2022, increase the number of Certified Peer Recovery Coaches from 1 to 5 as evidenced by MDHHS certification. Hire, train and certify coaches.
G2O1: By 7/31/2020, EHI will accept 50 more eligible referrals for PRSS, increasing referrals from 320 to 370 as evidenced by participant records. Expand program admissions.
G2O2: Provide PRSS coaching sessions to 100% of program participants over the grant period as measured by participant records. Provide peer coaching sessions.
G3O1: Each year, Case Managers and/or Certified Peer Support Specialists will assess 100% of EHI outpatient, intensive outpatient and residential (non-incarcerated) clients for eligibility to participate in PRSS. Assess all EHI clients for PRSS eligibility.
G3O2: Each year, link 50% of participants to employment support i.e. job search, resume develop, and interview techniques. Provide wraparound services.
G3O3: Link 100% of participants to formal and informal community resources over the grant period as measured by participant records. Provide case management.
G3O4: Each year, provide transportation as a wraparound service to 25% of participants without transportation to assess PRSS and/or linked community resources/services as evidenced by EHI transportation log. Provide transportation.
