The Passamaquoddy Health Center (PHC) Medication Assisted Treatment (MAT) Program will focus on improving care coordination for Community members most at risk, with a specific focus on populations with chronic disease, drug abuse and behavioral health conditions. This program will focus on activities that create a well-organized collaborative organization that includes medical providers, nurse care managers, behavioral health staff, pharmacy and Tribal Government staff, Tribal Citizens and support resources to accomplish this. The overriding objective of the PHC MAT program is to achieve the following:
1. Enable patients and populations at risk to overcome barriers to better health practices through appropriate medication treatment, case management, counseling (individual and group), education, support services and community based accessible resources.
Our programmatic goals are:
1. Provide a respectful supportive multidisciplinary team
2. Ensure that prescribed Buprenorphine is in accordance with DEA regulations.
3. Establish mechanism that will prevent drug diversion to the Tribal community.
