Community Opioid Recovery Expansion (CORE) - Munson Medical Center/Behavioral Health (MMC) is proposing CORE: “Community Opioid Recovery Expansion” within the 10-County Region of Northwest Michigan to include: Antrim, Benzie, Crawford, Grand Traverse, Kalkaska, Leelanau, Manistee, Missaukee, Roscommon, and Wexford. Sadly, Michigan is one of the 13 states in our nation with the highest number of prescriptions written for opioids. In 2015, the CDC reports that Michigan providers wrote a staggering 96.1 prescriptions for opioids per 100 people. We also have one of the highest rates of death from opioid and other drug overdoses. From 2010 – 2014, the CDC reports age adjusted deaths by overdose in our region at 17.1 per 100,000 compared to the national average of 15.6. From 2015-2016, Michigan saw a 19.6% increase in overdose deaths.
The goal of the MMC CORE is to build long-term recovery opportunities and improve health outcomes for individuals with Opioid Use Disorder by expanding access to MAT that includes care coordination with behavioral health, community, and Peer Recovery services. MMC’s population focus is low-income, rural, and underserved individuals. We will have open access to people seeking treatment and will include focus and outreach with two specialty populations: pregnant women with OUD and people with Substance Use Disorder (SUD) who present in crisis to the local Emergency Department. Over the 3-year project, 9/30/2018 – 9/29/2021; we will serve a minimum of 300 clients with evidence-based Opioid Use Disorder Treatment and will expand the model to other Munson Healthcare hospitals.
To accomplish this goal, MMC will complete the following project objectives:
1. MMC will finalize a MAT expansion plan, recruit/train staff members, and implement full program plan by January 1, 2019.
2. MMC will provide efficient access to OUD treatment to include MAT by qualified medical providers, behavioral health specialists, and Peer support services within 48 hours of the request for care by January 1, 2019.
3. Demonstrate a 75% decrease in illicit opioid drug use or opioid prescription misuse between baseline and six month visit.
4. MMC will complete a project evaluation assessing client outcomes, program goals, and implementing quality improvements by September 29, 2019 and subsequent years thereafter.
5. MMC will produce a sustainability plan to support the project goals of providing on-going evidenced-based care for the OUD population beyond the grant period by September 29, 2020.
6. MMC will develop and share program models that will be replicated in other areas of need in the 10-County Region by September 29, 2021.
MMC proposes to serve the following numbers of unduplicated clients within the next 3 years:
Year 1: MMC will serve a minimum of 80 unduplicated individuals with OUD
Year 2: MMC will serve a minimum of 100 unduplicated individuals with OUD
Year 3: MMC will serve a minimum of 120 unduplicated individuals with OUD
