BHcare witnessed a more than 57 percent increase in overdose deaths within its eleven-town service area in South Central CT since 2015, and as the state designated Local Mental Health Authority for the area, it is uniquely qualified to help change that statistic. BHcare proposes to aggressively expand its pilot Medication Assisted Therapy (MAT) program at its licensed and accredited outpatient clinics in Branford and Ansonia, Connecticut. Comprehensive treatment and supports will be provided for adults aged 18 years and older who are struggling with opioid use disorder (OUD), or OUD and a co-occurring mental illness. Treatment will include the prescription of buprenorphine/naloxone products, a comprehensive assessment, private, comfortable induction rooms; outpatient (weekly) and Intensive Outpatient (3 days per /week for 4-6 weeks) therapies; medication maintenance visits with the MD or APRN; as well as access to case management and peer recovery support, employment services, housing supports, social rehabilitation, care coordination and family supports - to provide a "whole-person" approach to address issues related to opioid dependence, including withdrawal, cravings and relapse prevention. Program participants will also have access to alternative therapies such as acupuncture and acupressure. BHcare will serve 500 individuals over the lifetime of the grant. Goal: To increase the number of individuals with OUD receiving MAT and decrease illicit opioid drug use and prescription misuse. Objective: 100 new clients will receive MAT in the first year (36 weeks) and 200 in year 2 and 200 year 3 as measured by admission and discharge data. Objective 2: A minimum of 80% of clients will receive a six-month follow-up GPRA interview. Objective 3: 50% of clients served will have abstinence from illicit drug use and alcohol misuse as measured by GPRA assessments, self-report, family / partner report, urine screenings, and breathalyzer assessments. Objective 4: 60% of clients served will have no criminal justice system involvement (or additional criminal issues) as measured by GPRA assessments, self-report, family/partner report. Objective 5: 50% of non-disabled clients served will be employed as measured by GPRA assessment, self-report, family/partner report. Objective 6: 60% of clients served will have stable housing as measured by GPRA assessments, self-report, family/partner report. Objective 7: 60% of clients served will have increased social connectedness, in at least two areas such as community groups, social clubs, recreational activities, church attendance, friends, family, etc. as measured by GPRA assessments, self-report, family/partner report. Objective 8: 80% Positive client satisfaction as reported through client satisfaction survey.
