Farnham Inc., Expansion of MAT for OUD Treatment in Oswego County - Project Name: Farnham Expansion of MAT to serve people with OUD in Oswego County Farnham Inc. will expand Medication Assisted Treatment (MAT) Services to serve people with Opiate Use Disorder in the rural county of Oswego, New York (NY). Oswego County has significant need including high levels of overdose, overdose deaths and relapse. As a result of this project, access to MAT will be increased and illicit drug use will be decreased. The County is one of 62 in NY and is a large rural area bordered to the north by Lake Ontario and spread over 1,312 square miles. The county’s population (122,104) is predominately white, non-Hispanic (96%) with 61% living in the most rural areas. Oswego County is experiencing socioeconomic challenges, including a high poverty rates, food insecurities, high unemployment rate, and low education attainment. Treatment admissions for opiate addiction are up 300% since 2010 and Oswego County has the 2nd highest opioid overdose death rate in the region (17.4) and a much higher rate than NYS (11.7). Farnham will implement three phases of programming; Outreach & Engagement (O&E), Treatment and Retention and Follow Up. Certified peer advocates will be developed to help engage and retain people in treatment. The service delivery network will be enhanced for better linkage to MAT. Innovative strategies like telehealth and jail based services will connect more people to treatment. Options for MAT will be expanded to include all 3 of the federally support medication options and capacity will be expanded. Evidence based psychosocial services will be implemented and include the ability to serve co-occurring disorders. These activities will accomplish the project goals of increasing the number of individuals with OUD receiving MAT and decreasing illicit opioid drug use and prescription opioid misuse by program participants. Project objectives include increasing psychosocial services, reaching the jail population who are ready for release, increasing referrals for MAT, increasing treatment retention rates, better serving co-occurring disorders and enhancing recovery support activities. As a result of these grant funds this project will retain 120 existing individuals with OUD and serve an additional 150 individuals in the first year of the project (270 served in year one). An additional 200 individuals will receive services in subsequent years serving 470 un-duplicated individuals over the course of the project.
