The King County Medication Assisted Treatment Expansion in the Community (KC-MAT-EPIC) program will prevent overdose, decrease opiate-user related deaths, and increase the capacity of local health agencies to better assist individuals with opioid use disorder (OUD) by expanding low barrier client-centered MAT capacity in key service areas at strategic locations where people with OUD already receive services. To further enhance treatment effectiveness, MAT will be delivered via nurse care managers using the Collaborative Care (CC) model, an evidence-based practice for the management of mental health conditions that has successfully been applied to substance use disorder (SUD) treatment. This model ensures patient-centered, trauma-informed care appropriate for the most vulnerable patients with the most complex needs, while effectively utilizing the time of physicians prescribing buprenorphine. The KC-MAT-EPIC grant anticipates serving 150 unduplicated individuals with OUD annually, for a total of 450 individuals over the course of the grant. The measurable objectives of the program are as follows: Goal 1: Make MAT stabilization access available in withdrawal management modality through the CC NCM model. •G1 Objective1: Identify and serve 100 individuals with OUD with MAT per year for the life of the grant. •G1 Objective 2: Link 90% of 100 people per year served with a care navigator for ongoing MAT, behavioral health and primary care support. •G1 Objective 3: Screen 95% of individuals with OUD that present for WM for eligibility to receive MAT. Goal 2: Increase community-based MAT access for individuals with OUD. •G2 Objective 1: Within four months of grant notification have two community providers establish programs to provide MAT at locations where people with OUD access care. •G2 Objective 2: Community locations serve 50 unduplicated people per year with MAT •G2 Objective 3: Each community location will connect 100% of individuals receiving MAT identified as needing behavioral health, recovery support, and primary care services to the appropriate service. Goal 3: Identify emerging practices of increased engagement, retention and success. •G3 Objective 1: Perform GPRA follow up with 80% of individuals served in the KC-MAT-EPIC grant. •G3 Objective 2: By end of grant year 1, develop ongoing continuous quality improvement process, including how data will be reported.
