The proposed project “Increasing Capacity of Medication Assisted Treatment (MAT) for Opioid Use Disorder Patients in Plymouth County” will provide increased medical and nursing intervention, as well as developmentally appropriate therapeutic interventions for patients aged 14 through adulthood. The project will work to enhance and create a comprehensive network of providers to support new patients obtaining a referral to MAT to reduce risk of relapse and death. The enhanced program will be able to increase capacity and support referrals for MAT in two outpatient programs in Plymouth County for adult patients (aged 18 and older) from community-based resources, including local Emergency Departments, primary care physicians, internal substance use disorder treatment programs (i.e.: detox, residential, inpatient psychiatric), local opioid coalitions, and pre-release incarcerated individuals. Additional enhancement of services will include increased MAT capacity and access for youth and young adults, aged 14 to 17, along with developmentally appropriate clinical interventions. The proposed project would provide increased access of MAT services to 200 patients over the course of the three-year project, including the capacity to provide services to 50 new unduplicated patients within Year One of the project, and 75 patients during Years Two and Three based on provider capacity. The project will implement a more comprehensive screening, assessment, and induction protocol for patients seeking MAT, which will assist with increased access, timeliness of induction, and integrated clinical services. The Nurse Case Management (NCM) model will be used for both outpatient locations to improve access, care coordination, and screening and assessment of patients, and psycho-education for all MAT referrals. Prescribers will provide medication management, including but not limited to MAT (Vivitrol, Subutex, Buprenorphine, oral Naltrexone) and psychopharmacology for co-occurring symptomology. Clinical services will be provided to all patients enrolled in the MAT program by a trained and qualified clinician utilizing Evidence-Based Practices (EBP) for the adult patients will include Motivational Interviewing (MI), Solution Focused Therapy (SFT), and Stages of Change Model, Cognitive Behavioral Therapy (CBT). Youth and young adult patients, aged 14 to 24, will be provided with Adolescent Community Reinforcement Approach (A-CRA) clinical services by a trained and certified clinician. The overall goals and objectives of the project would increase prescriber access for youth, young adults, and adults seeking MAT to treatment and prevent risk related to opioid use disorder. By adding increased access to MAT-waivered prescribers and building capacity for treatment, the overall objectives would be to reduce drop-out rates and relapse to less than 30% of the patient being treated, reduce the number of inpatient treatment admissions over a three year period of the project, and achieve a 90% engagement rate among youth and young adults in the treatment model.
