Background: The population of focus is underserved American Indian (AI) adolescents (ages 12-17) and transitional aged youth (ages 18-25) and their families/primary caregivers with SUD and/or co-occurring substance abuse and mental health disorders. The geographic catchment area is the same as the Sisseton Indian Health Service (IHS) delivery area: Lake Traverse Reservation mainly located in northeastern SD, with 60% of its land area being Roberts County. The local IHS unit Total User Population is 7,010 and 26% are 10-24 years old; over half (52.7%) of the Sisseton-Wahpeton Oyate (SWO) are under age 25 according to IHS Resource Patient Management System (RPMS) data in community needs assessment for SWO Tribal Action Health Plan (TAP) 2016-2020. Lake Traverse Reservation is a geographically isolated large area with small population centers in seven distant political districts. Food and housing insecurity, poverty, transportation and limited access to services exist with disproportionately high rates of mental health/substance abuse disorders and suicide compared to Whites living in the same area. Problem Statement: SWO is the first tribe in the Great Plains Area to develop a TAP pursuant to the Tribal Law and Order Act of 2010 and SWO Tribal Council seeking to deal with the high rates of crime, violence, and substance abuse characterized as a behavioral health crisis among SWO community and families. Youth are often the identified patient or symptom-bearers in family systems with parents or caregivers who struggle with mental health and substance abuse disorders; this can either lead to poor treatment and recovery outcomes for youth or it can facilitate change for family members and caregivers presented with the need to adapt healthier family functioning, parenting skills and support environment for the youth in treatment/recovery. Proposed Action: TREE will build SWO capacity to address the Tribal Council's declared behavioral health crisis (in adopting Controlled Substance Act of 2017) by expediting a solid overarching Behavioral Health response structure with enhanced, expanded and well-integrated services. 450 unduplicated adolescents and youth will receive comprehensive treatment, early intervention, and recovery support services for substance abuse disorders and/or co-occurring mental disorders, including family/caregiver and tobacco use interventions. Project partners include Dakotah Pride Treatment Center, Aliive Roberts County, Veterans Memorial Youth Center and Tribal Education Department, Schools, Court and Probation documented by MOUs, Letters of Commitment, and Collaborative Agreement SD-CA-09-006. SWO Tribal Council Resolution No. 18-03 authorizes TREE application to SAMHSA and commits its responsibility for implementation, management, data collection, performance monitoring and business accounting systems. TREE will increase access utilizing electronic health record system for integrated treatment and case management within the expanded, coordinated multi-system family-centered approach using evidence-based practices and services conducted at schools, youth center, intensive outpatient and day treatment settings to include pregnant young women.
