Native American Connections in partnership with its network of qualified direct service providers will provide a coordinated multi-system family centered approach to enhance and expand comprehensive evidence-based outpatient treatment, early intervention, and recovery support services among Transitional-Aged Youth (ages 16-25)with substance use/co-occurring disorders and their families, with particular emphasis on Native American youth disconnected from services, and/or homeless youth/families placed in emergency housing/shelters without access to healthcare in Maricopa County, Arizona. Maricopa County is home to an estimated 4.3 million residents (U.S. Census Quick Facts, 7/1/17). The estimated number of transitional aged youth population (ages 16-25) in Maricopa County in 2010 was close to 545,000 (14%) of total population (U.S Census 2010). TAY populations in Arizona, Maricopa County, as well within Arizona’s reservation and urban American Indian/Native American populations, experience significant incidence of substance abuse use, and other health issues: * 16.1% (5,524 individuals)treated for substance use disorders in Maricopa County were between ages 15-24 * Arizona's American Indian residents ranked worse than the statewide average on 49 of 65 health indicators 2015),all contributing to the premature death rate of 77.4 percent; 16 years younger at time of death compared to all racial/ethnic groups. NAC's Youth and Family TREE project will use a culturally competent, holistic, strengths-based, trauma informed, outcome focused, and systems oriented approach to accomplish the project’s goals/objectives. CSAT grant-funded services will be delivered within the context of either a Child and Family Team Model(wraparound services for 16-17 year olds) or Adult Recovery Team Model (wraparound services for 18 -25 year olds), with the focus on assisting the youth and family in their recovery journey. NAC's proposed project will implement these models in partnership with a strong network of Maricopa County based collaborative partners also dedicated to meeting the needs of the proposed TAY population of focus including: Phoenix Indian Center (urban Native Americans), UMOM New Day Center (homeless), OneNTen (LGBTQ, homeless) Maricopa County Correctional Health System (justice-involved with SUD), and Phoenix Children's Hospital (healthcare for homeless youth). The project will enhance and expand comprehensive evidence-based outpatient treatment, early intervention, and recovery support services to transition-aged youth and their family members within NAC’s full continuum of care, focused especially on early intervention and supporting individuals in recovery. NAC already operates a Transition Age Youth (TAY) program to assist youth to learn the skills needed to live independently. NAC offers a complete outpatient continuum of care starting with general behavioral health services or psychiatric services, case management, intensive outpatient services (office/community-based or home-based)and office-based group counseling and psychoeducation. The project will serve 50 youth/families in Year One and 100 youth/families annually in Years 2-5.
