Montgomery County has the highest recidivism rate among Ohio's six largest urban counties, with offenders returning to the county with the same barriers that they had prior to incarceration. The Montgomery County Offender Reentry Program (ORP) will address the needs of returning citizens by expanding and enhancing ORP services through transition planning and screening of substance use disorders (SUD) and co-occurring disorders (COD) prior to release, referral to treatment immediately at release, and pre-/post-release peer mentoring, case management, and systems linkages. The Substance Abuse Resources and Disability Issues (SARDI) Program in the Boonshoft School of Medicine at Wright State University will partner with the Montgomery County Office of Reentry, Montgomery County Jail (MCJ), the MonDay Community Correctional Institute, the County Sheriff's Office, and two community-based SUD treatment programs, Samaritan Behavioral Health, Inc. (SBHI) and Addiction Services (AS), to deliver this ORP in Montgomery County. At the time of incarceration, more than 75% of offenders at the MCJ and MonDay abused drugs and/or alcohol, and most of these offenders currently return to the county without any plans for SUD treatment. The goals of this Montgomery County ORP are: 1) reduce recidivism among offenders with SUD in MCJ and MonDay by providing pre-release and post-release services, including SUD/COD assessment, treatment and recovery services; 2) reduce HIV and hepatitis (Hep) risk and treatment disparities for ex-offenders released from MCJ and MonDay through prevention education, testing, and referral to treatment; 3) reduce recidivism in project participants by employing the RNR Model to address the criminogenic needs of high-risk ex-offenders, including screening, transitional planning, and wraparound case management; and 4) develop and maintain stakeholder partnerships that will plan, create and provide a transition from incarceration to community-based SUD treatment and reentry services, as well as plan for expanded opioid prevention programs for ex-offenders. Evidence-based, trauma-informed SUD treatment and recovery approaches will be used, including intensive, out-patient treatment, MI, CBT, MAT, PSRS, and 12-Step. Returning citizens (55% male, 53.5% African-American, 45.5% White), 18 years or older, who screen positive for SUD/COD, will receive case management, assessment, and appropriate SUD and mental health treatment from one of two contracted providers immediately upon release from MCJ or MonDay. This ORP will provide annually: 1) SUD screening to at least 200 and assessment to 150 ex-offenders who screen positive for SUD pre-release; 2) medication-assisted treatment (MAT) to a minimum of 100 ex-offenders; 3) intensive outpatient treatment to a minimum of 150 ex-offenders; 4) case management to a minimum of 150 ex-offenders; and 5) HIV and Hep testing to a minimum of 100 ex-offenders. This project is expected to support Montgomery County's goal to reduce recidivism by increasing access to services and producing better outcomes for offenders with SUD and co-occurring disorders at MCJ and MonDay.
