Casa Esperanza Inc.’s proposed “Transcend: Empowerment, Recovery, and Reentry” program will aim to reduce relapse and recidivism among Latino reentering citizens living with co-occurring substance abuse and mental health disorders (CODs) by expanding access to substance use disorder treatment and related recovery and reentry services. Over 5 years, Transcend will serve 240 individuals. Transcend’s goals are: 1) Support successful transition from incarceration to integrated substance abuse and mental health treatment to increase retention of, and improve long-term outcomes for, individuals reentering the community; 2) Provide treatment services that educate and empower participants to develop skills to identify risky behaviors for relapse and recidivism; solve problems and navigate complex systems through the development of self-efficacy and social skills; and identify resources to overcome barriers to reintegration; 3) Grow our Latino Reentry Network to expand access to an integrated recovery and reentry-oriented continuum of care focused on the unique needs of multiply-diagnosed Latino reentering citizens; and 4) Create and deliver culturally-focused Peer Leadership Development program to develop returning citizens into emerging Latino recovery leaders who promote healthy attachment and culturally relevant, trauma-informed, and recovery-oriented values in the community. Transcend will be anchored in the evidence-based practice of Integrated Dual Disorder Treatment (IDDT). IDDT is a framework that incorporates a powerful combination of individual, group and family evidence-based interventions within it. IDDT has a strong evidence base as an effective treatment for individuals with co-occurring substance use and mental health disorders; and is beginning to be tested with specific reentry populations, with promising results, especially for increased utilization of mental health services and reduced hospitalizations.
