Avivo proposes an expansion of its evidence-based continuum of SUD/COD treatment-based re-entry services and support—currently serving two state prisons—to include an additional four state prisons and two county correctional facilities. This expansion will target release violators and short-term offenders; increase the reach of services by 200 percent; and feature system linkages with state and county corrections as well as county Workforce Centers. According to 2016 Minnesota Department of Corrections (MN DOC) data, the two facilities currently served house a combined annual population of over 1,500. Leveraging longstanding partnerships, Avivo will expand services to four additional state facilities and one county correctional facility that, according to the same data, house a combined annual population of over 5,329. Nearly 50 percent of the total population to be reached through this expansion will be people of color. Eighty-five to ninety percent of Minnesota’s prison population has a SUD or COD, per MN DOC data, but only 30 percent of males and 60 percent of females get the treatment they need. All services will be provided by Avivo, a comprehensive mental and chemical health clinic providing licensed residential and outpatient substance use disorder and co-occurring treatment, among other services. Avivo’s pre-/post-release continuum incorporates multiple evidence-based practices, including Integrated Dual Diagnosis Treatment; Motivational Interviewing; Trauma-Informed Care; Risk-Needs-Responsivity; Core Correctional Practices; and A New Direction (AND), an evidence-based, cognitive-behavioral curriculum that targets criminal thinking. Avivo’s proposed expansion will achieve the following goals and outcomes: GOAL 1: Increase access to post-release SUD/COD treatment; OUTCOMES: 70% enrolled will complete treatment, 80% completing treatment will report improved mental/chemical health, 60% who complete program will not relapse; GOAL 2: Improve individuals’ abilities to change criminal behavior patterns through cognitive behavior; OUTCOMES: 80% enrolled will complete AND, 80% who complete AND will not violate release terms or be re-arrested, 75% who complete program will not recidivate; GOAL 3: Increase access to post-treatment support; OUTCOMES: 85% enrolled will execute individualized employment plan & take steps to achieve goals, 75% of these will find employment, 20% will gain industry-recognized credentials; 100% enrolled will access housing resources; 75% enrolled will access health/wellbeing services; 100% enrolled will connect to community-based & peer support; 80% will report increased support networks & connection to community.
