La Clínica del Pueblo proposes to expand and enhance its substance abuse treatment program to serve a larger and more geographically diverse number of Limited English Proficient Latinos at risk for and/or living with HIV in the target population of YMSM, MSM, and Transgender while enhancing the existing program to add viral hepatitis screening and education and a trauma-informed care approach to the program model (TREM). The program will build on the existing, integrated program, currently based on the Matrix Model for substance abuse treatment, and includes case management, mental health evaluation and treatment, HIV testing and education as integral program components. LCDP’s proposed program will ensure access to high-quality and accessible evidence-based substance abuse treatment, HIV and Viral Hepatitis testing and treatment services for 150 high-risk Latinos over the course of the grant period. Founded in 1983 by Central American refugees, La Clínica del Pueblo provides comprehensive primary care, social services and enabling, mental health, and substance abuse services to a largely Hispanic/Latino population from the Washington DC Metro Area. La Clínica has operated as a Federally Qualified Health Center (FQHC) since 2007 and a Patient Centered Medical Home since 2012.
