The Latino Commission on AIDS' “Our Health, Our Community/Nuestra Salud, Nuestra Comunidad” project will contribute to increase engagement in care for Latino YMSM (ages 18-29) and Latino MSM (ages 30 and older) with substance use disorders (SUD) and/or co-occurring substance use and mental disorders (COD) who are at risk for HIV or HIV positive that receive HIV services/treatment. The project aims to contribute to the nation’s achievement of the 90-90-90 goals regarding HIV status and treatment. The project will implement activities to ensure that clients are linked to care for SUD and/or COD treatment needs, recovery supportive services, smoking cessation referrals, HIV testing and case management services, patient navigation, case management, linkage and provision of HIV care and treatment, hepatitis and STI screening, referrals for housing services & legal services and other services identified by our participants. Another important aim is to identify SUD/COD among clients currently receiving services by enhancing and expanding services targeted to Latino YMSM and MSM to ensure they are receiving SUD/COD services if needed. Additionally, decreasing substance abuse, risk behavior and preventing HIV and SUD/COD among high-risk Latino YMSM and Latino MSM through community level, group-level and individual-level interventions and services centered around specific cultural and language needs. All Latino YMSM and MSM living with and at risk for HIV will receive specific services including individual screening (SBIRT) and counseling, referral and linkage to substance use and mental health treatment and other needs identified by each participant. Screening will be completed through SBIRT (Screening, Brief Intervention, and Referral to Treatment), an evidence-based practice used to identify, reduce, and prevent problematic use, abuse, and dependence on alcohol and illicit drugs. SBIRT is an early intervention approach that targets those with nondependent substance use to provide effective strategies for intervention prior to the need for more extensive or specialized treatment. Other Evidence Based Practices (EBPs) to be used are behavioral interventions that have been used to target at-risk populations with substance use experiences. They include, Community PROMISE, an HIV prevention intervention that has been adapted to reach populations at risk and living with HIV and at risk for SUD/COD; a group-level (Safety Counts) and individual level interventions (MIP, PCC and RESPECT) have been designed to assure that clients are involved in comprehensive integrated care and provided with referrals as needed. The expected outcomes include increasing the number of individuals with SUD/COD who are HIV positive that are on antiretroviral therapy (ART) linked to HIV Care, reducing the impact of behavioral health problems, reducing HIV risk and incidence, and increasing access to and retention in treatment for individuals with co-existing behavioral health, HIV and hepatitis co-conditions. This program will ensure that individuals who have been diagnosed with SUD and/or COD and HIV positive or at risk for HIV/AIDS have access and receive appropriate health services.
