Detroit Recovery Project’s proposed program, HEART, will annually serve 100 (500 over the life of the project) HIV-infected or at high risk for HIV infection living in Detroit, Michigan: racial/ethnic minority men who have sex with men – especially black and Latino young men (ages 18-29) – gay, bisexual, and transgender individuals with substance use disorders – especially people who inject drugs and/or who have co-occurring mental health and substance use disorders. HEART will provide onsite rapid HIV and hepatitis testing with referral to treatment as needed and case management with individualized support and services including housing assistance. It will use two evidence-based interventions and a complementary best practice of peer navigators, all of which will work together seamlessly to 1) identify and engage individuals in the high-risk target population who do not know their HIV status, 2) assist those recently diagnosed and unsure of next steps, and 3) re-engage those who for whatever reason disconnected from care. Detroit Recovery Project has identified 2 goals and their related objectives: Goal 1: Increase the number of PLWH with SUD/COD who are linked to HIV care. Objective 1: Institute an outreach campaign that results in 100 members of the target population to seek HIV Rapid Testing at DRP annually, 500 over the life of the project. Objective 2: Provide effective case management service that results in 95% those who test positive with HIV Rapid Testing to complete confirmatory testing and 90% of those confirmed positive to begin antiretroviral therapy (ART). Objective 3: Provide access to supports and services that results in 90% of those who begin ART to remain in care at least 24 months, through the follow-up period. Goal 2: Increase the number of PLWH with SUD/COD with improved health outcomes. Objective 1: Provide viral hepatitis testing to 95% of clients meeting USPSTF testing recommendations. Objective 2: Provide referrals and linkages to follow-up care and treatment for 95% of clients infected with viral hepatitis (B or C). Objective 3: Provide effective case management service that results in 75% clients referred to follow-up care and treatment to remain in care throughout program participation. Objective 4: Engage 90% of clients in SUD/COD treatment and/or recovery support services. Objective 5: Provide effective case management services that results in 70% of clients who smoke to enter a smoking cessation program, 50% of those who participate in such a program to report that they have decreased the average number of cigarettes smoked, and 20% to report that they have quit smoking.
