Project Name: Grants to Benefit Homeless Individuals- Treatment Services for Homeless LGBTQ Youth Project Summary: The Ali Forney Center, the largest provider of housing and supportive services for homeless LGBTQ youth in the nation, proposes the Treatment Services for Homeless LGBTQ to expand connections for youth to substance use disorder treatment and treatment for co-occurring substance use and mental health disorders, as well as case management and wrap-around services to increase access to permanent housing. Population of Focus: The population of focus will be 16-24 year old homeless or unstably housed LGBTQ youth, predominately men who have sex men (MSM) and racial/ethnic minorities. Interventions: The program will expand treatment for substance use disorders and co-occurring mental health disorders as well as expand access to permanent supportive housing utilizing a collaborative service approach. Key interventions will be outreach, treatment services, intensive case management, and wrap-around supportive services. Goals: : (1) Increase the number of homeless youth placed in permanent housing; (2) Increase the number of homeless youth engaged in comprehensive treatment and recovery-oriented services for behavioral health; (3) Expand the array of services available and the number of homeless LGBTQ youth engaged in care through increased collaboration with multiple organizations across NYC. Measurable Objectives: • Identify 100 homeless youth with substance abuse and/or co-occurring disorders to enter into program (500 over project lifetime) • Connect 100 youth with substance abuse counseling (500 over project lifetime) • Connect 100 youth to therapy and mental health counseling (500 over the project lifetime) • Develop an individualized housing plan for 100 youth to increase access to permanent housing (500 over project lifetime) • Increase the number of youth abstaining from substance use • Increase the number of homeless youth accessing supportive services at our Drop-In Center and through community-based partners
