Project Helping the Homeless Overcome Mental Exclusion (HOME) - Special Health Resources for Texas (SHRT), a nonprofit 501(c)(3) in East Texas, in partnership with Community HealthCORE and Hiway 80 Rescue Mission, proposes to implement Project Helping the Homeless Overcome Mental Exclusion (HOME) at three of its clinic locations located in Longview, Tyler, and Texarkana, Texas. Project Helping the Homeless Overcome Mental Exclusion (HOME) will target adult individuals, youth, and families experiencing homelessness and are affected by SUD or COD. The overall purpose of Project Helping the Homeless Overcome Mental Exclusion (HOME) is to develop and expand community infrastructures to integrate trauma-informed behavioral health treatment and support services for individuals with substance use disorders or co-occurring mental health and substance use disorders while securing permanent housing. SHRT will serve as the lead agency for this project providing Intensive Outpatient Treatment, coordination of care, and linkage to insurance and other benefit programs; HealthCORE, the local mental health and intellectual disability governing authority for the target area, providing co-occurring disorder treatment services; and Hiway 80 Rescue Mission, a housing support agency providing emergency shelter, housing assistance, disability and/or insurance benefits, employment training and assistance, financial management, and transportation. The overall goals of the program are: 1) to strengthen the partnerships capacity and infrastructure to ensure effective planning and service delivery to the target population; 2) to increase access to, or placement in, sustainable permanent housing, treatment and recovery support services, and Medicaid and other benefit programs; and 3) to evaluate and measure project accountability and impact. Project Helping the Homeless Overcome Mental Exclusion (HOME) will initiate implementation by the fourth month of funding. The number of clients served will be 250 for the life of the program (50 per year for five years). SHRT’s service delivery strategies and program development activities adhere to CLAS standards. Multiple evidence-based interventions will be delivered as part of the project to create a fully integrated array of services to meet the varied needs of the homeless populations and will include: NIDA Community Outreach Model – for outreach; Assertive Community Treatment – for screening, assessment, behavioral health treatment, case management, and recovery support; Cognitive Behavioral Therapy – for behavioral health treatment; and Seeking Safety – for trauma-informed model substance abuse treatment. SHRT is also licensed and funded by the State of Texas to provide outpatient treatment services to youth and is funded by the Center for Substance Abuse Treatment to provide treatment services to adults. A comprehensive Evaluation Plan will be adopted by Project Helping the Homeless Overcome Mental Exclusion (HOME) which includes clear performance and outcome measures and data management procedures to ensure expeditious and reliable collection, processing, analyses, and reporting of client- and program-level data.
