Through the Grants to Benefit Homeless Individuals (GBHI) program, Harbor Homes, Inc. (HHI) proposes to enhance existing housing, mental health, and substance use treatment services and supports in southern NH by building a comprehensive and integrative recovery and treatment system for homeless individuals. The goal of this program is to increase the number of homeless youth (ages 18-24) and families, and homeless individuals of all ages with a history of emergencies/crises due to chronic substance use disorder/ co-occurring disorder in New Hampshire, that become engaged in permanent housing that supports recovery and treatment for substance use disorders and co-occurring disorders, and encourages a transition to self-sufficiency. HHI’s long track record of implementing complex and holistic services for homeless individuals as well as their state of the art federally qualified healthcare for the homeless clinic and evaluation partnership with JSI, Inc. ensures a rapid start-up within four months of grant award and a high quality implementation and evaluation process. Over five years, HHI proposes to 1) enhance and integrate existing housing, behavioral health, primary care, and substance use treatment services and supports in southern New Hampshire to build a more comprehensive recovery-oriented system of care; 2) provide outreach to 450 homeless individuals and families to connect them with permanent housing, mainstream social services, and benefit programs over 5 years; and 3) provide wrap-around case management and recovery-oriented supports to 270 individuals living in permanent supportive housing to treat behavioral health and/or substance use disorders. Recognizing that recovery occurs via multiple pathways, HHI's GBHI program is flexible and responds to the unique needs of each individual. HHI maintains over 700 permanent housing units annually through other funding sources and commits to providing permanent housing and supportive services to 40 individuals in Year 1; 60 in Years 2, 3 and 4; and 50 in Year 5. Grant-funded staff will utilize evidence-based practices including the permanent supportive housing/ Housing First model, integrated dual disorder treatment, motivational interviewing, cognitive behavioral therapy, Critical Time Intervention, and supportive employment, using a trauma-informed, wraparound process, to meet the needs of the population of focus. HHI will collaborate with Keystone Hall, NH's premier substance use disorder treatment, prevention, and recovery support services center for the provision of intensive outpatient and high-intensity inpatient treatment.
