Summary – The Native American Rehabilitation Association of the Northwest (NARA), through its proposed Housing Support Services project, will develop and implement community and agency infrastructures that integrate culturally specific behavioral health and medical treatment, community-based permanent housing, and a range of wraparound supportive services for urban Native Americans. Population and demographics – The population of focus is American Indians and Alaska Natives (AI/AN) in the Portland, Oregon metropolitan area who are in need of permanent supportive housing and who experience multiple medical and behavioral impairments. Housing Support Services will serve 70 individuals (30 in year one, with 10 individuals added each of the next four years for a total of 70) who will receive permanent supportive housing, medical and behavioral health care, and wraparound recovery services. Housing Support Services will screen at least 50 individuals each year of the grant for a total of 250 over the five years, and will refer all eligible individuals for services through the local Coordinated Access system. NARA serves urban AI/AN who, as a demographic group, suffer from a very high rate of behavioral health disorders. NARA Clinic data from 2013 indicate approximately 23% of AI/AN patients had a diagnosis of depression or other mood disorder and approximately 15% of AI/AN patients were diagnosed with anxiety disorders or PTSD. (Health Resource Services Administration, 2013) According to the 2015 Point-In-Time count, more than half of Multnomah County’s homeless population reported having a disabling condition. Almost 800 (36%) reported a mental health condition, and over 1,000 (49%) reported substance abuse problems. Strategies/interventions – NARA and its community partners will: (1) use the local Coordinated Access system to identify and refer AI/AN; (2) place individuals in permanent housing that is supported with wraparound services and recovery assistance. (3) provide a primary medical home for each participant, arrange for in-house, culturally specific behavioral health treatment, medical and dental care; and (4) align housing efforts under one NARA Housing Office. Goals and Outcomes – Housing Support Services will: (1) expand case management capacity; (2) maintain housing placements; (3) provide medical homes and integrated care; (4) assist participants in achieving self-sufficiency; (5) develop agency housing infrastructure; (6) align efforts with Coordinated Access System; (8) partner with providers of affordable housing; (7) develop internal communication system; and (8) improve use of NARA’s data system. Anticipated outcomes are: connect with AI/AN individuals with housing and behavioral health treatment; support housing permanency; and provide wraparound recovery services and culturally specific supports to promote recovery and stability.
