PROJECT ABSTRACT
The Community Rehabilitation Center (CRC), established in 1993 and located in the heart of Jacksonville, FL, seeks funding for Project SAFE HORIZON with the purpose of integrating behavioral health, permanent housing, and recovery-based support services for individuals, youth and families in the community experiencing homelessness. Project SAFE HORIZON will emphasize collocation of intensive outpatient treatment, coordination of housing services, and recovery-based support services including securing health insurance and other benefits for the target population. CRC mostly serves the Health Zone 1, an area riddled with medical, behavioral, and socio-economical disadvantages among its mostly minority population living in poverty. The Purpose of SAFE HORIZONS is to expand CRC’s current behavioral treatment, permanent housing, and support services to serve individuals, youth, and families living in homelessness and to integrate a compendium of services available at CRC to serve homeless individuals in full coordination. Project SAFE HORIZON will fund the following services: trauma-informed outpatient treatment for 55 persons per year; total of 275 individuals, Cooccurring disorder screening and treatment, outreach, case management to secure permanent housing, health insurance, and benefits, recovery support services including transportation, skills training, employment, etc. In addition, CRC will maximize its resources via coordinated case management and refer individuals with identified needs to the following existing in-house services as needed (grant funds will not be utilized for these services): HIV and hepatitis screening, Primary health care and HIV-medical care, Permanent Housing, SOAR: SSI/SSDI Outreach, Access, and Recovery program. The Goals of the project are: 1) To develop capacity and infrastructure for the delivery of integrated, collocated services, to homeless individuals, youth, and families with a substance use disorders or co-occurring mental health and substance use disorders; 2) Implement integrated services and supports designed to reduce homelessness among individuals, youth and families who experience SUD and COD; and 3) Monitor progress and performance on achieving the goals and objectives of Project. Project SAFE HORIZON plans to enroll 245 clients for the life of the program (40 year 1, 55 year 2, 55 year 3, 55 year 4, and 40 year 5). In line with SAMHSA’s and the proposed project, multiple evidence-based interventions will be delivered as part of the project to create a fully integrated array of services: NIDA Community Outreach Model, Matrix Model – Intensive Outpatient Treatment for substance abuse, Cognitive Behavioral Therapy –behavioral health treatment, and Seeking Safety. – trauma-informed model. A comprehensive Evaluation Plan will be adopted by Project SAFE HORIZON which includes clear performance and outcome measures and data management procedures to ensure expeditious and reliable collection, processing, analyses, and reporting of program data.
