The Opioid State Targeted Response (STR) Supplement will leverage collaboration in communities hit hardest by the opioid epidemic in WV, the state having the highest overdose death rate in the nation, with trends demonstrating worsening of the opioid drug overdose death rates. The Supplement will establish an innovative, yet evidence-based approach to not only identifying individuals with substance use disorders (SUDs) in hospital emergency departments, but initiating immediate medication assisted treatment, utilization of peer recovery coaches and referral to community based treatment and recovery services under Project Engage. Furthermore, once identified and treated through Project Engage, individuals will be referred to PRO-ACT, a unique community-based "one-stop shop" to keep them in treatment, as well as offer a comprehensive set of evidence-based services to support individuals throughout the recovery period. This model will also be replicated in additional hospitals as part of this project. In addition, due to the very rural nature of West Virginia, a telehealth model for this approach will also be developed in partnership with a hospital in a small, rural, contiguous county. Goals for the WV STR Supplement include: Goal 1: Establish an integrated Project Engage service model of care in the Emergency Department that identifies individuals with substance use disorders to enable provision and coordination of immediate treatment, support and recovery services. Goal 2. Establish the infrastructure and operationalize PRO-ACT, as an innovative and cutting-edge healthy-oriented, community-based model, for individuals open and committed to change. Goal 3: Expand the integrated Project Engage/PRO-ACT service model of care to Kanawha County, including Thomas Health System, Charleston Area Medical Center, and PRO-ACT facility to identify individuals with substance use disorders to enable provision and coordination of immediate treatment, support and recovery services. Goal 4: Develop an integrated ED-based addiction treatment and recovery Project Engage tele-health service model for rural, underserved areas of WV. Goal 5: Increase treatment access (including medication assisted treatment) and prioritize access into treatment and recovery services for individuals with OUDs who present to emergency rooms. Goal 6: Increase treatment commitment and adherence by reducing wait time between initial request for services and enrollment in treatment, providing incentives and supports for continuing with treatment well-beyond the fourth session. The WV STR Supplement will support the successful, long-term recovery of individual with SUDs and reduce opioid overdose death rates by building and strengthening the infrastructure that supports their identification and recovery. The WV STR Supplement will further strengthen the system in the state to replicate and further expand the Project Engage/PRO-ACT model.
