Bay Mills Indian Community Health Center Prevention Program - Bay Mills Indian Community Health Center Prevention Program will provide Prevention Program services to six rural townships 1) Bay Mills Township, 2) Chippewa Township, 3) Superior Township, 4) Whitefish Township, 5) Dafter Township, 6) Hulbert Township surrounding Bay Mills Indian Community, but also to the greater populations of Chippewa County (36,081 people, 1,558.51 sq. miles), Luce (6,339 people, 899.08 sq. miles) and Mackinac Counties (11,236 people, 1,021.57 sq. miles) populations. BMHC Prevention Program will utilize Traditionally Adapted and Community Defined Evidence Based Practices in its programming to revitalize and embrace traditional practices using the Tribal Behavioral Health Agenda, SAMHSA Strategic Priorities and Make America Healthy Again goals as aims. Along with Traditionally Adapted and Community Defined Evidence Based Practices, traditional western Interventions, namely Mental Health Therapists will be utilized to further increase resiliency of youth and young adults - preventing substance use / misuse, overdoses, and suicides among AI/AN youth and young adults. BMHC Prevention Program aligns with SAMHSA's Strategic Priorities – 1. Preventing substance use and overdose. 2. Enhancing access to suicide prevention and mental health services. 3. Promoting resilience and emotional health for children, youth, and families. 4. Integrating behavioral and physical health care. 5. Strengthening the behavioral health workforce. Bay Mills Health Center Prevention Program will be available to youth (8 years old) through young adult (24 years old), on site at 12455 W. Lakeshore Dr, Brimley, Michigan, 49715, with ancillary recovery / prevention support services (Peer Recovery / Community Recovery Center), located at 3406 S. Pine Village Rd, Brimley, MI 49715-9324, two blocks from main health center. Telehealth psychological and behavioral health case management / counseling services will also be available to clients with access to Psychiatric services for Severe Mental Illness diagnosis. Referrals to additional onsite health services include medical, dental, vision, nursing, community health and referral care coordinators for outside specialty services (inpatient care, oral surgeons, etc.). BMHC offers sliding fee services, patient navigators, outreach specialists and case managers to assist clients in accessing health care. In addition to Using an accredited FQHC Patient Centered Medical Home model of care, using Trauma Informed Care practices, including traditional medicine and traditional practices with allopathic medicine to offer participants tailored, individualized therapy driven by participants.
