Multicultural AIDS Coalition proposes to implement the Health System Navigation project to provide comprehensive navigation services to Black and Latinx Boston residence who are at highest risk for HIV and substance use disorders. The project will engage members of the populations of focus in designing community education model with messages that resonate with those we seek to recruit into screening, navigation to medical and substance use treatment, and ongoing support until they have achieved self-efficacy in managing their health. The prioritized communities are (1) gay, bisexual, and other men who have sex with men; (2) people who inject drugs and others who misuse substances; (3) persons who exchange sex for money or survival; and (4) transgender women. Annually, the project will reach at least 2500 individuals with education messages, for a total of 10,000 for the 5-year duration of the project. 600 individual non-duplicated clients will receive HIV/substance use assessment/screening each year (3000 over 5 years); 400 (2000 over 5 years) will be members of the focus population; 200 (800 over 5 years) will be identified as having substance misuse or HIV challenges requiring some level of navigation support. The Goals of the project are to: A. Develop a Partner Network to increase substance misuse and HIV services to the population. B. Increase awareness of HIV/substance misuse risk by implementing an awareness campaign C. Increase access to HIV and substance misuse screening among focus populations D. Provide navigation to increase utilization of medical care and substance use treatment E. Increase retention of focus populations in medical care and substance use treatment Some of the Key Objectives are: 1. Annually provide HIV/HCV/STI screening to at least 400 members of focus populations. 2. Provide substance use assessment to 100% of clients screened. 3. Identify at least 200 persons annually who report 2 substance misuse risk factors for HIV 4. Provide navigation support to 100% of newly diagnosed or out-of-care HIV+ clients 5. Offer Ryan White services to 100% of HIV+ clients engaged, 6. Offer navigation to treatment services to 100% of clients with substance use disorder 7. Provide case management services to at least 30 high-acuity HIV positive clients 8. Provide case management/relapse prevention to at least 30 substance use disorder clients 9. Provide intensive group peer support to at least 32 HIV+ clients with histories of trauma and substance use challenges
