Project Name. Prevention Navigator Program (PNP). Whittier Street Health Center is proposing a Prevention Navigator Program that will provide services to those at highest risk for HIV and substance use disorders. The Prevention Navigator Program (PNP) at Whittier Street Health Center is designed to integrate our Substance abuse prevention and treatment services with the HIV/AIDS program. We currently have a robust behavioral health and substance abuse program which will be used to provide much needed services to high risk patients that will be screened and referred by Prevention Navigator staff members. Whittier is located at 1290 Tremont Street near the geographical center of Boston, and also manages a second clinic at Quincy Commons, a senior housing complex on 292 Blue Hill Ave, also in Roxbury. Both locations are in Suffolk County of Massachusetts which is among the hardest hit by the HIV epidemic. This County also has one of the highest substance abuse problems in the State. The Prevention Navigator program will utilize evidence-based practices SBIRT, naloxone and other medication assisted treatment (MAT) programs to provide services to the high risk substance abuse patients. Evidence-based behavioral interventions like ARTAS, Community Promise and HIV Counseling and Testing will be used to provide testing to patients who are at risk for contracting HIV/AIDS. Population and number of people to be served. The PNP program will serve 300 community members at high risk for substance abuse and HIV/AIDS. The patients will be screen and referred to our behavioral health department for comprehensive substance abuse services. 300 service area residents will be tested for HIV by the PNP staff and those that test positive for HIV will be immediately connected to treatment in our facility. All PNP staff members will be trained in Narcan administration every year. Utilizing Community Promise Intervention, PNP staff members will distribute 2,000 Role Model Stories every year. PNP staff members will distribute 20,000 condoms to individuals at in-reach and outreach events every year. Strategies and interventions. 1) Goal Reduce new HIV infections among targeted populations and increase the number of people who are aware of their HIV status. 2) Goal: Identify and effectively intervene with those who are at moderate or high risk for substance abuse and refer them to WSHC for comprehensive risk reduction counseling and treatment. 3) Goal: Increase patient education, screening, treatment options, care coordination and risk reduction for substance abuse clients. 4) Goal: Increase access for high risk patients with substance abuse problems to the Whittier substance abuse services including the medication assisted treatment program.
