Realistic Education in Action Coalition to foster Health (REACH LA) proposes the implementation of the Healthy Community Navigation Project for young African American and Latino MSM, ages 18-29, at high-risk for HIV and substance use disorders (SUD) in Los Angeles County (LAC). REACH LA will largely target members of the local African American House & Ball communities and Latino Party Crew Scene. REACH LA approach will integrate the provision of HIV Testing & Counseling services, Personalized Cognitive Counseling (PCC), Navigation & Peer Support services, Choosing Life: Empowerment! Action! Results! (CLEAR), and the implementation of the Social Network Strategy for HIV testing recruitment. Despite the advances in HIV prevention and treatment strategies, the burden of HIV on young African-American and Latino MSM still continues in LAC. In 2016, the HIV incidence among young MSM of color was 16% with a higher positivity rate among young Latino MSM (2.1%) and young African-American MSM (3.8%) as compared to the positivity observed among all testers across LAC (1.2%). Also, only 48% of young adults ages 18-29 were retained in care and 53% of them were virally suppressed as compared to the overall LAC retention in care and viral suppression proportions, respectively 54% and 60%. REACH LA will target LAC Service Planning Area (SPA) 4 (Metro), 6 (South), and 8 (South Bay). As of 2016, sixty-three percent of all PLWH – 61,000 people - live in the three targeted SPAs, and the highest rate of new HIV infections was among persons living in SPA 4 (48) followed by SPA 6 (32) and SPA 8 (17). Additionally, the 2018 NSDUH reported that among all adults 18 years and older in LAC, young adults 18-25 have significant higher use prevalence of various substances in the past year (Marijuana 32%, alcohol use 53.1%, alcohol SUD 10.5%), and higher need for alcohol use or illicit drug treatment but not receiving it (14.7% and 8.4%). Similar patterns also occurred within each of the three targeted SPAs. During the 5-year project, REACH LA will provide (a) HIV testing services to a minimum of 475 high-risk individuals from the target population (75 individuals in Year 1, and 100 individuals each year during Years 2-5), (b) Navigation & Peer Support Services to facilitate the linkage, engagement and retention of HIV-positive individuals in HIV care services, STD and viral hepatitis screening & treatment services, and when appropriate, substance misuse treatment services, and (c) CLEAR to a minimum of 70 high-risk and HIV-positive individuals from the target population (10 in Year 1, and 15 individuals each year during Years 2-5). Other key activities include the implementation of (1) community-based substance misuse and HIV prevention strategies, (2) Awareness Campaign on the risk of substance misuse among individuals living with HIV and the importance of seeking care & treatment, and (3) a competency-based training program to increase the capacity of SUD treatment and other health care providers to conduct screening for HIV & viral hepatitis, and develop and implement a care plan for HIV-positive young MSM.
