Sacramento County Public Health (SCPH) will build a robust Substance Abuse and HIV Prevention Navigation Program in order to better serve racial/ethnic minority youth, 13-24 years of age living in Sacramento County, resulting in a reduction in HIV and improved health outcomes related to substance use disorders (SUD). SCPH has determined that the project will deliver these services to the following populations of focus: racial/ethnic minority youth age 13-24 living in Sacramento County who are distinctly marginalized coming from the Foster Care System and Child Protective Services, homeless and runaway youth, youth at high-risk for SUD, and youth diagnosed with HIV not currently retained in care. The goal of the program is to serve 150 youth annually with testing for sexually transmitted infections including HIV, and screening for SUD. This project will provide testing/screening and prevention messaging to 750 youth over the duration of the five-year funding cycle. SCPH will partner with various community sectors including social services, county-level governmental agencies, and youth serving organizations to implement comprehensive community-based SUD and HIV prevention strategies. Specifically, SCPH will partner with Wind Youth Services, an agency serving homeless and runaway youth in Sacramento County; Sacramento Court Appointed Special Advocates, serving youth in the child welfare system; and the Sacramento County Division of Alcohol and Drug Services (ADS). SCPH will provide education and training to at least 30 SUD treatment and other healthcare providers annually, (30 x 5 yrs =150 providers total) on the importance of screening for HIV and assuring that clinical care needs are met if an individual is determined to be infected with HIV. SCPH staff will develop provider-focused educational resources for distribution throughout Sacramento County. Staff will also provide professional development opportunities to SUD treatment providers through regular presentations at the Sacramento County Opioid and Methamphetamine Coalition meetings, as well as the ADS Provider Meetings. Project staff will utilize two evidence-based interventions for this project. The CRAFFT tool will be used to screen for SUD among youth; and Seventeen Days, a video-based sexual health intervention, will be used to address sexual risk behaviors among youth. The efforts outlined above will result in improved health outcomes for youth in Sacramento County.
