Wabanaki Public Health (WPH) is a collaborative organization (created in state legislation in 2011, in partnership between the State of Maine and the tribal chiefs) that works with tribal and tribal non-profit organizations to deliver public health services to Wabanaki people (Maine tribal people). Each tribal nation partners with WPH, to deliver critical public health services, while identifying health inequities and addressing health disparities. WPH staff are employed by Wabanaki Health and Wellness (WHW). WHW handles all financial, human resources, and operational support. WHW is a tribal non-profit organization whose mission is to provide high-quality, culturally relevant services to Native Americans .The population of focus for this grant opportunity will be tribal citizens aged 13-24 residing on and off reservation lands. WPH and WHW understand the importance of addressing service gaps. For this project, we propose a project that will substantially increase the capacity of our HIV and HCV program to reach individuals ages 13-24 and refer them to services. WHW will be leveraging our strategic partnerships with Wabanaki Public Health to meet the needs of program participants. Our goals are aligned with the required activities to develop strategic partnership, network, link, and refer individuals to services, provide HIV and HCV testing services.
